AMFEPRAMONE
(DIETHYLPROPION)

I: Obesity.
CI: Advanced arteriosclerosis. Hyperthyroidism.
Hypersensitivity. Glaucoma. Agitated states.
History of drug abuse. Pulmonary HTN or severe
hypertension. Heart murmur or valvular disorders.
Emotional instability. History of psychiatric illness.
Concomitant use w/ other anorectic agents. MAOI
therapy or w/in 2 wk of discontinuation. Pregnancy
& lactation. Child & elderly.
SP: Increased risk of developing primary pulmonary
HTN (PPH). HTN & severe CV disease. Prolonged
use may induce dependence w/ withdrawal
symptoms on cessation of therapy; do not use for
more than 3 months. May increase convulsions in
some epileptic patients. Toxic psychosis. Peptic
ulceration. Prostatic hyperplasia.
AR: CV & CNS disturbances; GI effects; allergic
reactions; impotence; changes in libido,
gynecomastia, menstrual upset; hematopoietic
effects; dyspnea, hair loss, muscle pain, dysuria,
increased sweating, polyuria.
DI: Hypertensive risk w/ MAOIs. b-blockers,
guanethidine, tricyclic antidepressants, urinary
alkalinisers or acidifiers. Alters antidiabetic drug
requirements.
BUPRENORPHINE
I: As a perioperative analgesic supplement.

Moderate to severe pain. Premedication before
anaesthesia.
CI: Acute alcoholism; convulsive disorders; head

injuries; increased intracranial pressure; comatose
patients; resp depression & obstructive airway
disease; patients on established opioid agonists.
SP: Hepatic or renal disease; pregnancy, lactation;
hypothyroidism; adrenocortical insufficiency;
asthma; prostatic hyperplasia; shock; hypotension;
inflammatory or obstructive bowel disorders;
myasthaenia gravis; infants/neonates. Reduce dose
in elderly & debilitated patients. May precipitate
withdrawal symptoms in narcotic addicts.
AR: Sedation, nausea, dizziness, vertigo,
hypotension, miosis, headache, hypoventilation,
resp or CNS depression; tachycardia, bradycardia,
urinary retention, coma, vomiting, drowsiness,
sweating, confusion, dry mouth, diaphoresis,
withdrawal syndrome.
DI: Plasma-buprenorphine concentrations may be
affected when co-administered w/ drugs that
induce or inhibit cytochrome P450 isoenzyme
CYP3A4. Enhanced depressant effects w/ other
CNS depressants e.g. alcohol, anaesthetics,
anxiolytics, hypnotics, TCAs & antipsychotics.
Diazepam may produce resp & cardiac collapse.
CATHINE
(D-NORPSEUDOEPHEDRINE)
I: Obesity.
CI: Patients treated w/ MAOI or stopped MAOI w/in
the last 2 wk; moderate or severe hypertension;
pulmonary hypertension; structural cardiac
abnormalities or advanced arteriosclerosis; adrenal
tumour; hyperthyroidism or hyperexcitability; coronary
thrombosis; closed-angle glaucoma; pregnancy.
SP: Heart disease, mild hypertension, CHF or angina
of effort; prostate hyperplasia; patients w/ history
of drug or alcohol abuse; DM; psychotic patients; renal
disease. Growth retardation may occur in children.
AR: Headache, tachycardia, precordial pain,

DI: Reduced effect of guanethidine. Increased risk
of arrhythmia in patients treated w/ digitalis due to
its sympathomimetic effects. Increased risk of
arrhythmia w/ chloroform, cyclopropane, halothane
or other halogenated anesthetics. The effect of
cathine may be reduced/enhanced by tricyclic antidepressants. Cathine should not be used in patients
treated w/ a MAOI or w/in 14 days of stopping MAOI
as hypertensive crisis may occur if used.
EPHEDRINE

I: Relief of moderate to severe acute asthma, relief
of broncoconstiction.
CI: Hypersensitivity. Hypertension, thyrotoxicosis,
BPH. Lactation.
SP: Ischaemic heart disease, hyperthyroidism,
diabetes mellitus, hypertension, angle-closure
glaucoma, renal impairment; prostatic
enlargement; pregnancy, elderly.
AR: Anxiety, tachycardia, tremor, dry mouth,
hypertension, cardiac arrhythmias, impaired
circulation to the extremities, nervousness, insomnia,
palpitations. Difficulty in micturition in patients w/
prostatic enlargement. Nasal drops: Local irritation,
rebound nasal congestion & drug-induced rhinitis on
prolonged use. Delusions, hallucinations. Seen w/
hypersensitivity & overdosage. Acute CNS & CVS
stimulation presenting as vomiting, fever,
hypertension, psychosis. Cardiac arrhythmias.
DI: Reduces antihypertensive effect of bethanidine
& guanethidine. May increase clearance of
dexamethasone. Increased incidence of adverse
effects when used w/ theophylline. Severe HTN when
combined w/ MAOIs or within 2 wk of discontinuance
of MAOI treatment. Increased risk of arrhythmias w/
cardiac glycosides, quinidine or tricyclic
antidepressants. Increased vaso-constriction or
pressor effects w/ ergot alkaloids or oxytocin.
FLUNITRAZEPAM
I: Induction of anaesthesia. Premedication in

surgery. Short-term management of insomnia.
CI: Preexisting CNS depression or coma, resp

depression, acute pulmonary insufficiency or sleep
apnoea; severe hepatic impairment; chronic
psychosis, phobic or obsessional states; may
precipitate suicide or aggressive behavior; not to
be used alone to treat depression or anxiety
associated w/ depression; glaucoma. Pregnancy,
lactation. Neonates.
SP: Chronic pulmonary insufficiency; elderly or
debilitated patients; muscle weakness, impaired
liver or kidney function; drowsiness may affect
skilled tasks; monitor cardio-respiratory function
when used for deep sedation; personality disorders
or organic brain changes; history of alcohol or drug
addiction; resp depression & hypotension w/
parenteral admin; this route should only be used
when facilities for reversing resp depression are
available.
AR: Drowsiness & lightheadedness, sedation,
muscle weakness & ataxia; less frequently vertigo,
headache, confusion, depression, slurred
speech, changes in libido, tremor, visual
disturbances, urinary retention, GI disturbances,
changes in salivation & amnesia.
DI: Enhanced sedation or resp & CV depression
w/ drugs w/ CNS depressant properties eg,
antidepressants, antihistamines, antipsychotics,
general anaesthetics, other hypnotics or sedatives,
opioid analgesics & cisapride. Alcohol. Calciumchannel blockers; clonidine; diaxozide; disulfiram;
clozapine; diuretics; moxonidine; nabitone; nitrates;
cimetidine, esomeprazole & omeprazole; muscle
relaxants; dopaminergics; lofexidine; hydralazine,
minoxidil & nitroprusside; antifungals; angiotensin
II receptor antagonist; antiepileptics; -blockers;
antibacterials. Drugs which alter the hepatic
microsomal enzymes.
FLURAZEPAM
I: Short-term management of insomnia.

CI: Hypersensitivity; porphyria. Pregnancy (3 rd
trimester); neonates.
when used for deep sedation; personality disorders
or organic brain changes; history of alcohol or drug
addiction. Respiratory depression & hypotension
w/ parenteral admin. Dependence; lactation. Safety

recovery period. Chronic alcoholic & alcoholintoxicated patients. Preanaesthetic elevated
CSF pressure. Dependence & tolerance may
develop. May impair ability to drive or operate
machinery. Monitor cardiac function in patients
w/ HTN or cardiac decompensation. Pregnancy
& lactation.
AR: Emergence reactions e.g. vivid dreams,
hallucinations, confusion, irrational behaviour.
Increased muscle tone sometimes resembling
seizures. Temporary HTN & tachycardia.
Hypotension, bradycardia, arrhythmias.
Respiratory depression, apnoea, laryngospasm,
diplopia, nystagmus, nausea, vomiting,
lachrymation, hypersalivation, raised intraocular
& CSF pressure, skin rash & pain at inj site.
DI: May increase effects of nondepolarising
muscle relaxants. May reduce hypnotic effect of
thiopental. Prolonged recovery time w/
barbiturates/narcotics. Reduced cardiac output,
BP & pulse rate w/ halothane. Increased risk of
HTN & tachycardia w/ thyroid hormones.
Seizures & tachycardia may occur when used
w/ theophyllines.

LORMETAZEPAM

depression or associated anxiety; unstable

serious cardiac disorders that could increase the
risk of sudden death. Extended-release form:

SP: Pregnancy & lactation. History of alcohol or
drug abuse. Hypertension & other CV disorders
that might be exacerbated by increases in BP or
heart rate. Pre-existing psychosis or bipolar
disorder; monitor for symptoms of aggression or
hostility. History of seizure disorder. Children <6
yr (growth suppression); monitor growth during
therapy. May impair ability to drive or operate
machinery. Transdermal: Avoid exposure of
application site to any direct external heat source.

serum levels or effects w/ CYP2D6 inhibitors e.g.
chlorpromazine, delavirdine, fluoxetine,
miconazole, paroxetine, pergolide, quinidine,
quinine, ritonavir, ropinirole. May increase serum
levels of phenytoin, TCAs. Possible severe HTN &
tachycardia w/ sibutramine. CNS depression w/
alcohol. Severe toxic reactions w/ clonidine.
Increased risk of hypertensive crisis w/ MAOIs.

MIDAZOLAM

I: Induction of anaesthesia. Premedication in
surgery. Sedation for dental & minor surgical
procedures. Sedation in critical care. Short-term
management of insomnia.
CI: Acute narrow-angle glaucoma; coma or patients
in shock; acute alcohol intoxication; intrathecal &
epidural admin. Acute pulmonary insufficiency or
marked neuromuscular resp weakness including
unstable myasthenia gravis; severe resp
depression.
SP: Paediatric patients w/ CV instability; chronic
renal failure; open-angle glaucoma; cardiac
disease; resp disease; myasthenia gravis;
neonates; history of drug or alcohol abuse; elderly
& debilitated (reduce dose); avoid prolonged use
or abrupt withdrawal; hepatic impairment; severe
fluid or electrolyte disturbances. May impair ability
to drive or operate machinery; titrate dose carefully;
monitor for early signs of hypoventilation, airway
obstruction, or apnea. Pregnancy, lactation.
AR: Physical & psychological dependence w/
withdrawal symptoms; decreased tidal vol &

I: Anorectic in short-term treatment of moderate

CI: Pulmonary hypertension, advanced
arteriosclerosis, moderate to severe hypertension,
hyperthyroidism, CV disease, glaucoma, history
of alcohol/drug abuse, agitated states, use during
or w/in 14 days following MAOI therapy. Lactation.
SP: Hypertension, DM, seizure disorder, Tourette
s syndrome, patients who have used other
anorexigens w/in the past year, children. May
impair ability to drive & operate machinery. Potential
for drug dependency. Discontinue if results are
not satisfactory w/in 4 wk or tolerance develops.
History of epilepsy. Renal or hepatic disease.
Pregnancy.
AR: Urticaria, GI upsets, palpitations, tachycardia,
elevated blood pressure, overstimulation,
restlessness, insomnia, tremor, headache, dry
mouth, dizziness, dysphoria, euphoria, psychosis,
changes in libido & impotence. Primary pulmonary
HTN & valvular heart disease.
DI: Decreased excretion by antacids, carbonic
anhydrase inhibitors (e.g. acetazolamide,
dichlorphenamide, methazolamide). Excretion is

FENTANYL

I: Adjunct to general anaesthesia. Breakthrough
cancer pain. Intractable chronic pain. Premedication
before anaesthesia.
CI: Hypersensitivity.
SP: Myasthaenia gravis. Head injury; increased
intracranial pressure; intracranial lesions; renal or
hepatic impairment; neonates; opioid-nontolerant
patients. Increased risk of resp depression in

headache, sedation, transient hypotension,
peripheral vasodilation; increased intracranial
pressure. High IV dose may cause chest wall
rigidity. Transdermal: Rash, erythema & itching.
Respiratory depression, trunk rigidity,
laryngospasm, bronchoconstriction.
DI: Depressant effects may be enhanced by other
CNS depressants e.g. alcohol, anaesthetics,
anxiolytics, hypnotics, TCAs & antipsychotics.
Ammonium Cl may increase excretion of fentanyl.
Phenothiazines may increase hypotensive effect
of opioid analgesics. Serum levels may be
increased by CYP3A4 inhibitors (e.g. azole
antifungals, clarithromycin, diclofenac, doxycycline,
erythromycin, imatinib, isoniazid, nefazodone,
nicardipine, propofol, protease inhibitors, quinidine,
telithromycin & verapamil). May reduce the efficacy
of pegvisomant. Serum levels may be reduced by
rifamycin derivatives. May increase serotonergic
effect of SSRIs & sibutramine.
CI: Acute resp depression; acute bronchial asthma;

acute alcoholism; risk of paralytic ileus; raised
intracranial pressure or head injury.
SP: Dose should be individualised. Prolonged QT
interval; CV disease; hepatic impairment;
electrolyte abnormalities; concommitant use of QT
prolonging drug; resp disease; elderly. May impair
ability to drive or operate machinery.
AR: Nausea; vomiting; constipation; anorexia;
abdominal pain; drowsiness; resp depression;
hypotension; bradycardia; euphoria; headache;
dysphoria; urinary retention; miosis; visual
disturbances; impotence; dizziness; sweating;

Cardiac arrest; resp arrest; shock.
DI: Withdrawal symptoms may be experienced w/
naloxone, naltrexone, pentazocine, nalbuphine,
butorphanol, & buprenorphine. Increased
clearance & possible reduced efficacy w/ some
antiretrovirals (abacavir, amprenavir, efavirenz,
nelfinavir, nevirapine, ritonavir, lopinavir); rifampin;
phenytoin; phenobarbital; carbamazepine.
Decreased clearance & possibly increased toxicity
w/ CYP3A4 inhibitors (e.g. ketoconazole,
voriconazole, sertraline, fluvoxamine). Increased
chance of cardiac toxicity w/ other QT prolonging
drugs. Increased CNS depression w/ alcohol, other
opioids or CNS depressants. Methadone may
increase plasma levels of desipramine, zidovudine.
MORPHINE
I: Acute pain. Acute pulmonary oedema. Analgesia

during labour. Chronic pain. Intractable cough
associated w/ lung cancer. Moderate to severe
pain. Pain associated w/ myocardial infarction. Pain
relief. Premedication in surgery. Unstable angina
not responsive to anti-ischaemic therapy.

SP: Hepatic or renal diseases; head injury, CNS
depression, history of opioid agonist dependence;
biliary tract or acute abdominal disorders; Addison
s disease; obesity; urinary difficulties; resp
disease; thyroid disease; infants<3 mths,
debilitated patients & elderly; pregnancy.
AR: CNS depression, hypotension, drowsiness &
palpitation, bradycardia; sweating, flushing,
pruritis; malaise; urinary tract spasm & reduction
in urination; constipation; miosis. Severe resp
depression; increased intracranial pressure.
DI: Increased CNS depression w/ alcohol, other
opiate agonists, general anesthetics, tranquillizers,
sedatives & hypnotics. However, some
tranquillizers e.g. phenothiazines may antagonize
the effect of opiate analgesia; opium prep may
increase the effect of TCA & MAOIs. Reduced
effectiveness of diuretics in patients w/ CHF.
Should not be given to patients bitten by
Centruroides sculpturatus, Ewing & C. gertschi
Stahnke scorpions as opiate agonist may potentiate
the effect of the venoms. May cause serotonin
syndrome if used w/ MAOIs.
