Autologous bone marrow cell transplantation in acute spinal cord injury—an Indian pilot study

To determine the safety and feasibility of autologous bone marrow transplantation in patients wit... more To determine the safety and feasibility of autologous bone marrow transplantation in patients with acute spinal cord injury (SCI) via two routes of transplantation as compared with controls. Setting: Indian Spinal Injuries Center, New Delhi. Methods: Twenty-one subjects with acute, American Spinal Injury Association Impairment Scale (AIS) A (complete), traumatic SCI with neurological level T1-T12, were recruited and randomized into three groups of seven subjects each. Two groups underwent cell transplantation through the intrathecal or intralesional route, whereas the third served as control. Participants were assessed at baseline and followed up at 6 months and 12-months post enrollment. Safety and tolerability were evaluated by monitoring for any adverse events. Efficacy was assessed through neurological, functional and psychological evaluation, as well as through electrophysiological studies and urodynamics. Results: Surgery was tolerated well by all participants. There were no significant adverse events attributable to the procedure. There was no significant improvement in the neurological, electrophysiological or urodynamic efficacy variables. A statistically significant improvement in functional scores as evaluated by the Spinal Cord Independence Measure and International Spinal Cord Injury Scale was observed in all groups. Conclusions: The procedure is safe and feasible in AIS A participants with thoracic-level injuries at 12-months follow-up. No efficacy could be demonstrated that could be attributed to the procedure.

None of the previously published case series concerning this problem during the second trimester of pregnancy have explored the option of simultaneous surgical intervention for it along with maintenance of pregnancy. A 22-year-old woman with 26 weeks of pregnancy (2nd trimester) presented with upper back pain for the past 2 months, inability to move both lower limbs for the last 1 week, bladder and bowel dysfunction for the past 5 days (Frankel Grade B). Patient subsequently underwent MRI scan dorsal spine and the image findings were suggestive of spinal tuberculosis T2 level. After obstetric evaluation and opinion of the expectant mother, in view of extensive neurological deficit which progressed rapidly, decision was taken for surgical intervention along with maintenance of pregnancy. Chronic subdural haematoma (CSDH) is known to occur spontaneously in elder people causing symptom... more Chronic subdural haematoma (CSDH) is known to occur spontaneously in elder people causing symptoms of altered mental status, monoplegia, headache, seizure, falls and extra pyramidal symptoms[ 1 6 _ T D $ D I F F ]. 1 Trauma is an important factor in the development of CSDH. However, a history of head injury (direct trauma) is absent in about [ 1 7 _ T D $ D I F F ] 30-50% of the cases. Indirect trauma seems to be more important. About half the patients have a history of fall but without hitting their head on the ground[ 1 8 _ T D $ D I F F ]. 2,3 In many situations, the trauma is so trivial that it is forgotten. Even though patients are known to present with neurological deficits[ 1 9 _ T D $ D I F F ] , we could not find any reports of chronic subdural haematoma presenting with paraparesis. Here we report a case of [ 2 0 _ T D $ D I F F ] 58-year-old male patient presenting with paraparesis due to chronic subdural haematoma[ 2 1 _ T D $ D I F F ] , and review the literature about the causes and mechanism of neurological deficits in chronic subdural haematoma.

A 22-year-old woman with 26 weeks of pregnancy (2nd trimester) presented with upper back pain for the past 2 months, inability to move both lower limbs for the last 1 week, bladder and bowel dysfunction for the past 5 days (Frankel Grade B). Patient subsequently underwent MRI scan dorsal spine and the image findings were suggestive of spinal tuberculosis T2 level. After obstetric evaluation and opinion of the expectant mother, in view of extensive neurological deficit which progressed rapidly, decision was taken for surgical intervention along with maintenance of pregnancy. Patient was positioned in right lateral position after giving general anesthesia using double lumen endotracheal tube with lung isolation technique. Exposure was done using transthoracic third rib excision approach. Decompression was achieved by radical debridement at T2 vertebrae level followed by multiple rib strut grafts and stabilization with screw and rod construct between T1 and T3 vertebrae. Intra-operative measures including type of anesthesia, prevention of maternal hypotension, hypoxemia and hypothermia, and fetal monitoring by attending obstetrician were undertaken to maintain feto-maternal safety. Postoperative ultrasonography evaluation of the fetus revealed a normal study. Post-surgery histopathological evaluation of the surgical specimen confirmed tuberculosis infection and the patient continued anti-tubercular drug therapy for 9 months. She delivered a healthy girl child at 36 weeks of gestation by cesarean section. After about 14 months of postoperative follow-up, patient has completely recovered motor power with mild persistent sensory symptoms. She is self-voiding with mild constipation requiring occasional intermittent laxative use. Radiological improvements in comparison to the previous reports were also seen at the last follow-up. Although this is only a single case but being the first to our knowledge, the good results highlight the point that both surgical management and maintenance of pregnancy during second trimester complicated by Pott&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39;s paraplegia are possible, involving a multi-disciplinary team approach for optimal maternal and fetal outcome.

The purpose of this in vitro study was to evaluate the antimicrobial effects of five antibiotics ... more The purpose of this in vitro study was to evaluate the antimicrobial effects of five antibiotics when added to Kerr Pulp Canal Sealer EWT against Enterococcus faecalis. Five antibiotics: amoxicillin, penicillin, clindamycin, metronidazole, and doxycycline, were added separately to Kerr sealer. Thirty brain heart infusion agar plates were inoculated with E. faecalis and sterile paper discs containing a sealer-antibiotic combination were randomly assigned to the inoculated plates. Kerr sealer on a sterile paper disc and a blank sterile paper disc served as the controls. Fifteen plates were incubated aerobically and the remaining 15 plates were incubated anaerobically at 37 degrees C. The zones of inhibition were measured at 48 hours. No significant differences (p &amp;amp;amp;amp;gt; 0.05) were found between aerobic and anaerobic groups. The groups were combined and data analysis using a one-way ANOVA and Tukey&amp;amp;amp;amp;#39;s post hoc test was performed. Results revealed that sealer-antibiotic combinations containing amoxicillin, penicillin, clindamycin, and doxycycline had a significant difference (p &amp;amp;amp;amp;lt; 0.001) in the mean zones of inhibition when compared to Kerr EWT sealer alone. No significant differences (p &amp;amp;amp;amp;gt; 0.05) were found between the metronidazole-sealer combinations and Kerr EWT sealer alone.

To study the mechanism of hypercalciuria in metabolic acidosis, ammonium chloride loading (long) ... more To study the mechanism of hypercalciuria in metabolic acidosis, ammonium chloride loading (long) test was performed in 68 stone formers and 50 controls. Administration of ammonium chloride in both stone formers and controls produced a significant increase in urinary volume, ammonium and calcium excretions, no change in plasma calcium and creatinine clearance and significant decrease in plasma bicarbonate. However, on the third day of ammonium chloride loading test, urinary ammonium excretion and plasma bicarbonate levels were significantly lower while urinary calcium excretion was significantly greater in stone formers than in controls. Thus calciuresis could be correlated with the degree of metabolic acidosis but not with the rate of urinary ammonium excretion.

Although E undergoes hydrophobic interactions in W, specific interactions between E and W favour their mutual dissolution. The process of succussion (or trituration) of homoeopathic drugs with E (or lactose) produces potentized E or W molecules that mimic the properties of the drugs. Homoeopathic medicines have been postulated to operate via electron-transfer processes in the Central Nervous System. A mode of action of hom0eopathic medicines, consistent with the recent concepts of chemistry and physics is proposed to explain the scientific basis of the law of similars and some other characteristic pharmacologic actions of hom0eopathic medicines.

The variables studied were demographics, mode of trauma, neurological status, neurological improvement, involved vertebral level, duration of hospital stay, comorbid factors, expenditure and complications during the stay.Results:Forty-six patients with diagnosis of AS with spine trauma were admitted over the last 5 years with a total of 52 fractures. Under basal state, the stone formers showed statistically significant higher urinary pH, and lower 24-hour urinary titrable acid, ammonium and total hydrogen. On the 3rd day of ammonium chloride loading test, the stone formers showed statistically significant higher urinary pH and lower 24-hour urinary titrable acid, ammonium, total hydrogen and plasma bicarbonate. &#39;Clearance&#39; indices of hydrogen ion excretion were also depressed in a great majority of the stone formers. The results indicate the frequent co-existence of defective urinary acidification with nephrolithiasis.

Objectives: To determine the safety and feasibility of autologous olfactory mucosal transplantation into the spinal cord in chronic spinal cord injured using the technique developed by Carlos Lima et al. Setting: Spinal Injury Center, New Delhi. Methods: Five chronic, motor complete, traumatic spinal cord injury (SCI) patients with neurological level C5-T12 underwent the procedure. Participants were assessed at baseline and at 6 monthly intervals. Safety and tolerability were evaluated through monitoring for any adverse events and tests including magnetic resonance imaging (MRI) evaluation. Efficacy assessment was done through neurological, functional and psychological evaluation, electrophysiological studies and urodynamics. Results: Surgery was tolerated well by all American Spinal Injury Association (ASIA) Impairment Scale (AIS) A participants. The only AIS B participant lost sensory scores significantly after surgery but is gradually regaining it. MRI evaluation revealed a syrinx in one participant and increase in length of myelomalacia in four participants. There were no other adverse findings on MRI evaluation. There was no significant improvement in any of the neurological, electrophysiological or urodynamic efficacy variables. Statistically significant improvement was seen in functional scores as evaluated by Spinal Cord Independence Measure, Beck Depression Inventory scores and life impact scores on International Spinal Cord Injury Scale. Conclusions: The procedure is relatively safe and feasible in AIS A participants with thoracic level injuries at 18 month follow-up. No efficacy could be demonstrated which could be attributed to the procedure.

To develop a web-based educational resource for health professionals responsible for the manageme... more To develop a web-based educational resource for health professionals responsible for the management of spinal cord injury (SCI). The resource: www.elearnSCI.org is comprised of seven learning modules, each subdivided into various submodules. Six of the seven modules address the educational needs of all disciplines involved in comprehensive SCI management. The seventh module addresses prevention of SCI. Each submodule includes an overview, activities, self-assessment questions and references. Development of the resource: Three hundred and thirty-two experts from The International Spinal Cord Society (ISCoS) and various affiliated societies from 36 countries were involved in developing the resource through 28 subcommittees. The content of each submodule was reviewed and approved by the Education and Scientific Committees of ISCoS and finally by an Editorial Committee of 23 experts. Key features: The content of the learning modules is relevant to students and to new as well as experienced SCI healthcare professionals. The content is applicable globally, has received consumer input and is available at no cost. The material is presented on a website underpinned by a sophisticated content-management system, which allows easy maintenance and ready update of all the content. The resource conforms to key principles of e-learning, including appropriateness of curriculum, engagement of learners, innovative approaches, effective learning, ease of use, inclusion, assessment, coherence, consistency, transparency, cost effectiveness and feedback. Conclusion: www.elearnSCI.org provides a cost effective way of training healthcare professionals that goes beyond the textbook and traditional face-to-face teaching.

Introduction: We report the case of a woman with spinal cord injury (SCI) who presented to us wit... more Introduction: We report the case of a woman with spinal cord injury (SCI) who presented to us with Fournier's gangrene. Case report: A 60-year-old patient with SCI, ASIA A, neurological level D6, on clean intermittent catheterization presented with rapid necrosis of the perineal region, including the labia and anus, which developed after traumatic catheterizations for clean intermittent catheterization. She required repeated debridement and loop colostomy for management. Conclusion: We conclude that patients with SCI are rarely at risk for Fournier's gangrene secondary to neurogenic bladder and bowel, as well as to impaired sensations and genital flora. Treating physicians need to be aware of this complication in order to prevent mortality.

In this study for the first time, we compared physico-chemical and biological properties of polyc... more In this study for the first time, we compared physico-chemical and biological properties of polycaprolactonegelatin-hydroxyapatite scaffolds of two types: one in which the nano-hydroxyapatite (n-HA) was deposited on the surface of electrospun polycaprolactone-gelatin (PCG) fibers via alternate soaking process (PCG-HA AS) and other in which hydroxyapatite (HA) powders were blended in electrospinning solution of PCG (PCG-HA B). The microstructure of fibers was examined by scanning electron microscopy (SEM) and transmission electron microscopy (TEM) which showed n-HA particles on the surface of the PCG-HA AS scaffold and embedded HA particles in the interior of the PCG-HA B fibers. PCG-HA AS fibers exhibited the better Young's moduli and tensile strength as compared to PCG-HA B fibers. Biological properties such as cell proliferation, cell attachment and alkaline phosphatase activity (ALP) were determined by growing human osteosarcoma cells (MG-63) over the scaffolds. Cell proliferation and confocal results clearly indicated that the presence of hydroxyapatite on the surface of the PCG-HA AS scaffold promoted better cellular adhesion and proliferation as compared to PCG-HA B scaffold. ALP activity was also observed better in alternate soaked PCG scaffold as compared to PCG-HA B scaffold. Mechanical strength and biological properties clearly demonstrate that surface deposited HA scaffold prepared by alternate soaking method may find application in bone tissue engineering.

A 25-year-old male presented with a history of sudden onset of complete quadriplegia with sensory loss below the neck along with loss of bowel and bladder control. He had no history of any constitutional symptoms. He reported 10 days later. He was managed conservatively and after two weeks of intensive rehabilitation he had complete neural recovery. The spontaneous recovery of neurological impairment is attributed to the spreading of the hematoma throughout the epidural space, thus decreasing the pressure with partial neural recovery. Conservative treatment is a fair option in young patients who present late and show neurological improvement. The neurological status on presentation will guide the further approach to management.

She is self-voiding with mild constipation requiring occasional intermittent laxative use. Radiological improvements in comparison to the previous reports were also seen at the last follow-up. Although this is only a single case but being the first to our knowledge, the good results highlight the point that both surgical management and maintenance of pregnancy during second trimester complicated by Pott&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39;s paraplegia are possible, involving a multi-disciplinary team approach for optimal maternal and fetal outcome.

Five antibiotics: amoxicillin, penicillin, clindamycin, metronidazole, and doxycycline, were added separately to Kerr sealer. Thirty brain heart infusion agar plates were inoculated with E. faecalis and sterile paper discs containing a sealer-antibiotic combination were randomly assigned to the inoculated plates. Kerr sealer on a sterile paper disc and a blank sterile paper disc served as the controls. Fifteen plates were incubated aerobically and the remaining 15 plates were incubated anaerobically at 37 degrees C. The zones of inhibition were measured at 48 hours. No significant differences (p &amp;amp;amp;amp;gt; 0.05) were found between aerobic and anaerobic groups. The groups were combined and data analysis using a one-way ANOVA and Tukey&amp;amp;amp;amp;#39;s post hoc test was performed. Results revealed that sealer-antibiotic combinations containing amoxicillin, penicillin, clindamycin, and doxycycline had a significant difference (p &amp;amp;amp;amp;lt; 0.001) in the mean zones of inhibition when compared to Kerr EWT sealer alone. No significant differences (p &amp;amp;amp;amp;gt; 0.05) were found between the metronidazole-sealer combinations and Kerr EWT sealer alone.

Administration of ammonium chloride in both stone formers and controls produced a significant increase in urinary volume, ammonium and calcium excretions, no change in plasma calcium and creatinine clearance and significant decrease in plasma bicarbonate. However, on the third day of ammonium chloride loading test, urinary ammonium excretion and plasma bicarbonate levels were significantly lower while urinary calcium excretion was significantly greater in stone formers than in controls. Thus calciuresis could be correlated with the degree of metabolic acidosis but not with the rate of urinary ammonium excretion.

Homoeopathic medicines have been postulated to operate via electron-transfer processes in the Central Nervous System. Under basal state, the stone formers showed statistically significant higher urinary pH, and lower 24-hour urinary titrable acid, ammonium and total hydrogen. On the 3rd day of ammonium chloride loading test, the stone formers showed statistically significant higher urinary pH and lower 24-hour urinary titrable acid, ammonium, total hydrogen and plasma bicarbonate. &#39;Clearance&#39; indices of hydrogen ion excretion were also depressed in a great majority of the stone formers. The results indicate the frequent co-existence of defective urinary acidification with nephrolithiasis.

Objectives: To determine the safety and feasibility of autologous olfactory mucosal transplantation into the spinal cord in chronic spinal cord injured using the technique developed by Carlos Lima et al. Setting: Spinal Injury Center, New Delhi. Methods: Five chronic, motor complete, traumatic spinal cord injury (SCI) patients with neurological level C5-T12 underwent the procedure. Participants were assessed at baseline and at 6 monthly intervals. Safety and tolerability were evaluated through monitoring for any adverse events and tests including magnetic resonance imaging (MRI) evaluation. Efficacy assessment was done through neurological, functional and psychological evaluation, electrophysiological studies and urodynamics. Results: Surgery was tolerated well by all American Spinal Injury Association (ASIA) Impairment Scale (AIS) A participants. The only AIS B participant lost sensory scores significantly after surgery but is gradually regaining it. MRI evaluation revealed a syrinx in one participant and increase in length of myelomalacia in four participants. There were no other adverse findings on MRI evaluation. There was no significant improvement in any of the neurological, electrophysiological or urodynamic efficacy variables. Statistically significant improvement was seen in functional scores as evaluated by Spinal Cord Independence Measure, Beck Depression Inventory scores and life impact scores on International Spinal Cord Injury Scale. Conclusions: The procedure is relatively safe and feasible in AIS A participants with thoracic level injuries at 18 month follow-up. No efficacy could be demonstrated which could be attributed to the procedure.

The resource: www.elearnSCI.org is comprised of seven learning modules, each subdivided into various submodules. Six of the seven modules address the educational needs of all disciplines involved in comprehensive SCI management. The seventh module addresses prevention of SCI. Each submodule includes an overview, activities, self-assessment questions and references. Development of the resource: Three hundred and thirty-two experts from The International Spinal Cord Society (ISCoS) and various affiliated societies from 36 countries were involved in developing the resource through 28 subcommittees. The content of each submodule was reviewed and approved by the Education and Scientific Committees of ISCoS and finally by an Editorial Committee of 23 experts. Key features: The content of the learning modules is relevant to students and to new as well as experienced SCI healthcare professionals. The content is applicable globally, has received consumer input and is available at no cost. The material is presented on a website underpinned by a sophisticated content-management system, which allows easy maintenance and ready update of all the content. The resource conforms to key principles of e-learning, including appropriateness of curriculum, engagement of learners, innovative approaches, effective learning, ease of use, inclusion, assessment, coherence, consistency, transparency, cost effectiveness and feedback. Conclusion: www.elearnSCI.org provides a cost effective way of training healthcare professionals that goes beyond the textbook and traditional face-to-face teaching.

Case report: A 60-year-old patient with SCI, ASIA A, neurological level D6, on clean intermittent catheterization presented with rapid necrosis of the perineal region, including the labia and anus, which developed after traumatic catheterizations for clean intermittent catheterization. She required repeated debridement and loop colostomy for management. Conclusion: We conclude that patients with SCI are rarely at risk for Fournier's gangrene secondary to neurogenic bladder and bowel, as well as to impaired sensations and genital flora. Treating physicians need to be aware of this complication in order to prevent mortality.

In this study for the first time, we compared physico-chemical and biological properties of polyc... more In this study for the first time, we compared physico-chemical and biological properties of polycaprolactonegelatin-hydroxyapatite scaffolds of two types: one in which the nano-hydroxyapatite (n-HA) was deposited on the surface of electrospun polycaprolactone-gelatin (PCG) fibers via alternate soaking process (PCG-HA AS) and other in which hydroxyapatite (HA) powders were blended in electrospinning solution of PCG (PCG-HA B). The microstructure of fibers was examined by scanning electron microscopy (SEM) and transmission electron microscopy (TEM) which showed n-HA particles on the surface of the PCG-HA AS scaffold and embedded HA particles in the interior of the PCG-HA B fibers. PCG-HA AS fibers exhibited the better Young's moduli and tensile strength as compared to PCG-HA B fibers. Biological properties such as cell proliferation, cell attachment and alkaline phosphatase activity (ALP) were determined by growing human osteosarcoma cells (MG-63) over the scaffolds. Cell proliferation and confocal results clearly indicated that the presence of hydroxyapatite on the surface of the PCG-HA AS scaffold promoted better cellular adhesion and proliferation as compared to PCG-HA B scaffold. ALP activity was also observed better in alternate soaked PCG scaffold as compared to PCG-HA B scaffold. Mechanical strength and biological properties clearly demonstrate that surface deposited HA scaffold prepared by alternate soaking method may find application in bone tissue engineering.

A 25-year-old male presented with a history of sudden onset of complete quadriplegia with sensory loss below the neck along with loss of bowel and bladder control. He had no history of any constitutional symptoms. He reported 10 days later. He was managed conservatively and after two weeks of intensive rehabilitation he had complete neural recovery. The spontaneous recovery of neurological impairment is attributed to the spreading of the hematoma throughout the epidural space, thus decreasing the pressure with partial neural recovery. Conservative treatment is a fair option in young patients who present late and show neurological improvement. The neurological status on presentation will guide the further approach to management.
