In an effort to identify risk factors for insomnia and determine the contribution of nighttime ro... more In an effort to identify risk factors for insomnia and determine the contribution of nighttime road traffic volume to insomnia in the general population, a questionnaire survey was carried out among 3,600 adult Japanese women living in eight urban residential areas. The crude prevalence rate of insomnia was 11.2%. Multivariate analysis revealed that aging, living with a child/children aged six or younger, undergoing medical treatment, experiencing major life events, having an irregular bedtime, having a sleep apnealike symptom, and living near a road with a heavy volume of traffic are risk factors for insomnia. Taking into account other risk factors, there was a level-response relationship between the nighttime traffic volume of main roads and the risk of insomnia in the subjects living in the zones 0-20 m from these roads. These results suggest that road traffic noise raises the sound level in bedrooms in such zones, and consequently the prevalence rate of insomnia among the residents, and that noise-induced insomnia is an important public health problem, at least in highly urbanized areas. To confirm this, a further study on noise exposure is needed.

Self-administered questionnaires were distributed to 155 parents of disabled children living in A Prefecture in Japan. A total of 62 (40%) parents responded anonymously. The order of their expectations concerning care-givers for their children at schools was as follows; registered nurse, 22 (35%); mother, 19 (31%); homeroom-teacher, 15 (24%); school-nurses, 14 (23%); physician, 8 (13%); father, 7 (11%). This study also found registered nurses were expected to do medical care in collaboration with parents and teachers.
Abstract: In order to investigate the difference in diurnal variation of autonomic control betwee... more Abstract: In order to investigate the difference in diurnal variation of autonomic control between young male mild to moderate hypertensive and normotensive white-collar workers, we accessed heart rate variability with ambulatory blood pressure for over 24 hours including work and sleep periods. Both mean systolic and diastolic blood pressures (SBP and DBP) were significantly (p<0.01) higher in hypertensives (HT, n=11) than normotensives (NT, n=11) in all phases, whereas heart rate was not different between them in any phase. Blood pressures during work period (on-duty) were significantly higher than those during non-work period (off-duty) in both NT and HT. High frequency (HF; 0.15–0.40 Hz) power showed a circadian pattern in phase with the sleep-wake cycle. Both HF and low frequency (LF; 0.05–0.15 Hz) powers in HT were significantly lower than that in NT during the sleep period (p<0.05). Japan is a super-aging society, so nurses are expected to hold increasingly essential roles in bo... more Japan is a super-aging society, so nurses are expected to hold increasingly essential roles in both hospital-and community-based settings in the future. To strengthen community-oriented geriatric and community health nursing competencies among undergraduate nursing students, we implemented an undergraduate nursing practicum called the Preventive Home Visits Practicum (PHVP). In the PHVP, students are grouped into teams of four (a freshman, sophomore, junior, and senior) before visiting healthy community-dwelling older adults to discuss life, health, and activities of daily living. A qualitative descriptive focus group design was used to examine student learning outcomes across 4 years of participation in the PHVP. The data obtained in focus group interviews were descriptively analyzed. Students deepened their understanding of health and aging lives in an aging population and developed interpersonal relationships and communication through the practicum across the 4-year period. Additionally, we found that a team composed of students from all different grades achieved very effective learning through interactions among team members. These findings suggest that nursing practicums that require nursing students to visit community-dwelling older adults longitudinally, such as the PHVP, may have an important educational impact and may address the needs of the aging population.

Living near a road with a heavy traffic volume is one of the risk factors for insomnia. The risk for insomnia in the zones 0-20 m from the main roads increased linearly with the night-time traffic volume. This suggests that road traffic noise raises the sound level in bedrooms in such zones, and consequently the prevalence rate of insomnia among the residents, and that noise-induced insomnia is an important public health problem, at least in highly urbanized areas.
Risk for insomnia by nighttime road traffic noise invading into dwellings was investigated for 19... more Risk for insomnia by nighttime road traffic noise invading into dwellings was investigated for 19 pairs of case and control women in 2 areas with major road passing in Tokyo. A good correlation was observed between simultaneously measured indoor and outdoor sound levels in L Aeq (1 min) during sleeping time (n=14645: 7,241 for cases and 7,404 for controls: r=0.863; p<0.001), whose difference was about 25 dB on the average. The ORs by quartiles of indoor L Aeq (1 min) levels were 1.0 for <20 dB, 3.2 (95% CI: 2.9-3.6) for 29-34 dB, 3.6 (3.2-4.1) for 34-38 dB and 6.1 (5.5-6.8) for 38 dB or above (p<0.001 for trends), suggesting the minimum effective level for insomnia is around 30 dB in terms of L Aeq (1 min) during sleep.
