He was treated with co-trimoxazole and hydrocortisone. After 14 days of co-trimoxazole he had improved clinically and radiographically and was discharged receiving maintenance treatment. Comment Both these patients were positive for HIV antibody and had a clinical and radiological presentation suggestive of P carinni pneumonia that responded to treatment; thus the criteria fbr a diagnosis of AIDS were satisfied. ' In case 2 the diagnosis was complicated by the previous amiodarone treatment, though pneumonitis induced by amiodarone is unusual at a daily dosage less than 400 mg.23 Chest infections are common in the elderly. The increasing incidence of HIV infection and a reluctance of older homosexuals to admit to at risk behaviour may mean that elderly patients will die without AIDS being diagnosed. In the United States 10% of patients with AIDS are over 50, 2-5% over 60, and 0 4% over 70. The table shows the age distribution of our patients with AIDS up to October 1987. Six of 235 (2 5%) patients were over 60. P carinii pneumonia should be considered in all patients with atypical chest infections, even when there are no apparent risk factors. Hypertension in children and adolescents has gained ground in cardiovascular medicine, thanks to ... more Hypertension in children and adolescents has gained ground in cardiovascular medicine, thanks to the progress made in several areas of pathophysiological and clinical research. These guidelines represent a consensus among specialists involved in the detection and control of high blood pressure in children and adolescents. The guidelines synthesize a considerable amount of scientific data and clinical experience and represent best clinical wisdom upon which physicians, nurses and families should base their decisions. They call attention to the burden of hypertension in children and adolescents, and its contribution to the current epidemic of cardiovascular disease, these guidelines should encourage public policy makers, to develop a global effort to improve identification and treatment of high blood pressure among children and adolescents.

Resumen Los diuréticos tiazídicos son los fármacos más recomendados para el tratamiento inicial d... more Resumen Los diuréticos tiazídicos son los fármacos más recomendados para el tratamiento inicial de la hipertensión arterial, por su bajo coste, pero también por su efi cacia y tolerabilidad tanto en monoterapia como en combinación. Aunque el fármaco más utilizado es la hidroclorotiazida existen argumentos a favor de su sustitución por clortalidona. El uso de tiazidas reduce la incidencia de ictus y de enfermedad coronaria, como han demostrado numerosos ensayos clínicos. La combinación de tiazidas con otros antihipertensivos aumenta su efi cacia. El efecto adverso más común es la hipopotasemia, por lo que deben vigilarse los valores plasmáticos de potasio. Dado que muchos pacientes no alcanzan el objetivo terapéutico con un solo fármaco, si no existen contraindicaciones, debería incluirse un diurético en cualquier pauta terapéutica que asocie fármacos antihipertensivos.

The vascular (blood pressure, heart rate and peripheral blood flow) and uterine (spontaneous moti... more The vascular (blood pressure, heart rate and peripheral blood flow) and uterine (spontaneous motility) responses to intravenous methoxamine were studied in anaesthetized rats pre-treated with diethylstilboestrol. Methoxamine produced an increase (0.5-2 mg/kg) or did not modify (0.01 and 3 mg/kg) spontaneous uterine motility. The alpha 1-agonist also induced a hypertensive effect (0.1-3 mg/kg) accompanied by bradycardia at the highest doses, and a decrease in blood flow significantly greater in intestinal than uterine tissues. These effects were abolished by prazosin. The uterine action of methoxamine in vivo appears to result from the balance between myometrial alpha 1-excitatory effect and vascular alpha 1-vasoconstriction which induced uterine inhibition. The oestrogens appear to protect the alpha 1-mediated vasoconstriction.

Objective To describe the use of medicines and to determine the frequency of off-label use in eme... more Objective To describe the use of medicines and to determine the frequency of off-label use in emergency room paediatric patients. Patients and methods A prospective, observational and descriptive study was carried out in the setting of the paediatric emergency room of a Spanish general hospital. Medicines used by children <14 years prior to their emergency room visit were analysed based on information collected from parents/guardians and relatives for each drug prescription. Off-label use was defined as the utilization of a drug at an indication, dosage, frequency or route of administration that differed from the specifications in the Summary of Product Characteristics or by children outside the authorized age group. Results The patient cohort comprised 462 children, among whom 336 children had been prescribed 667 prescriptions. Of the medicines prescribed, 90% fell into only five 5 Anatomical Therapeutic Chemical Classification System groups. The most frequent active principles were ibuprofen and paracetamol. Of a total of 152 different formulations recorded, no paediatric information was provided for 40 formulations, and one formulation was contraindicated in children. Based on the established criteria, 338 prescriptions were off-label: no paediatric information or contraindication in children were available (82 prescriptions); the drug was used for an indication different from the authorized one (111 prescriptions); drug use was inconsistent with age recommendations (16 prescriptions); drug use was inconsistent with dose/frequency (129 prescriptions). Of the 152 formulations, 107 were occasionally used in an off-label manner. Conclusions Although the mean number of drugs used in children is small, off-label use is frequent. Research efforts should target paediatric studies that allow a rational drug use in children.
