Professor Sameh K. Morcos FRCS, DMRD, FFRRCSI, FRCR was a Consultant Radiologist at Sheffield Teaching Hospitals, UK. Retired in 2011 but retains his association with the University of Sheffield as Emeritus Professor of Diagnostic Imaging. Professor Morcos was the President of the European Society of Urogenital Radiology (ESUR) from 2006-08 and Secretary of its Contrast Media Safety Committee from 1994 to 2012. Professor Morcos was the associate editor of the British Journal of Radiology for more than 10 years and Deputy Editor of European Radiology from 2007 to 2010. [1] and Editor in Chief Dr. Robert Stanley's editorial [2] highlighting a possible causal relationship between gadodiamide and nephrogenic systemic fibrosis. These cases are in agreement with the findings of previous reports [3-6] and bring the total number of published cases of nephrogenic systemic fibrosis associated with gadodiamide to 47. These cases are probably the so-called "tip of an iceberg" because a significant number of unreported cases have been brought to our attention [7]. We have explored the possible mechanisms that may explain the association between gadodiamide and nephrogenic systemic fibrosis, and it becomes apparent that the stability of the chelate and the release of free gadolinium could be involved in triggering the fibrotic process [8, 9]. In recent reports, investigators have documented the presence of gadolinium atoms in skin biopsies of patients with nephrogenic systemic fibrosis [10, 11]. Of the extracellular chelates in clinical use, the least stable molecules in terms of the physiochemical properties are the nonionic linear chelates (i.e., gadodiamide and gadoversetamide) and the most stable molecules are the cyclic chelates. We agree with Dr. Stanley's recommendation that gadodiamide use should be strictly avoided in patients on dialysis and in patients with end-stage renal disease or acute hepatorenal syndrome. Gadolinium-containing contrast media for radiographic examinations: a position paper

Recently, it has been suggested that gadolinium-based contrast media could be used for radiologic... more Recently, it has been suggested that gadolinium-based contrast media could be used for radiological examinations in patients with significant renal impairment, previous severe generalized reaction to iodinated contrast media or thyroid disease about to undergo radioactive iodine treatment; however, the indications for and risks of using gadolinium agents in this way are not well known; hence, the Contrast Media Safety Committee of The European Society of Urogenital Radiology reviewed the literature to issue a position paper on this subject. A comprehensive literature review was performed and the resulting report was discussed at the Ninth European Symposium on Urogenital Radiology in Genoa, Italy, June 2002. Review of the literature indicates that according to experimental data on animals gadolinium-based contrast media have more nephrotoxic potential than iodinated contrast media in equivalent X-ray attenuating doses; therefore, gadolinium-based contrast media should not replace iodinated contrast media in patients with renal insufficiency for radio-graphic examinations. For patients with previous severe generalized reactions to iodinated contrast media, and in patients about to undergo thyroid treatment with radioactive iodine gadolinium-based contrast media in approved intravenous doses, up to 0.3 mmol/kg body weight will not give diagnostic radiographic information in most cases. Gadolinium-based contrast media are not approved for radiographic examinations.

The recent need for selective bronchography during bronchoscopy in certain patients, together wit... more The recent need for selective bronchography during bronchoscopy in certain patients, together with paediatric indications for tracheo-bronchography, raises the question as to which contrast medium is both safe and efficient. The purpose of the present experimental study was to define the tissue reaction caused by iotrolan and iopamidol in the bronchi and lungs of rats. 60 animals used in this study were divided into five groups receiving iotrolan 300, iopamidol 370, iopamidol 150, physiological saline and anaesthetic only, the last two acting as control groups. Statistically there was no significant difference in the histological reaction between iotrolan 300, iopamidol 370 and iopamidol 150 compared with the control groups, although iotrolan 300 appeared to result in the least tissue reaction. With its adequately high iodine concentration, low osmolality and relatively high viscosity, iotrolan 300 would appear to be a suitable contrast medium for bronchography.
