There are high chances of errors in diagnosis of CHD in adult population due to various reasons. If proper diagnosis is made, many of these CHDs are amenable to surgery leading to improved quality of life. Aims: Primary objective of the study was to know the detection rate of unrepaired CHD in adults. We have also reported common errors done while making the diagnosis of CHD in adults. Subjects and Methods: This was a retrospective observational study conducted from March 2020 to September 2021. Patients more than 18 years old who came for transthoracic echocardiogram (TTE) for any reason were included, and the detection rate of unrepaired CHD was calculated. The disease causes diminution of manpower and is of great public health signicance. We present a case series and retrospective analysis of seven children admitted to a tertiary care hospital of coastal Karnataka with a diagnosis of Brucellosis. Their epidemiological characteristics, contact history, clinical features and laboratory parameters were studied. This report aims to create awareness about the disease and highlight the importance of its consideration as a differential diagnosis of Pyrexia of unknown origin.

While the majority of reported cases are self-limiting with few lasting complications, a few deadly species can cause life-threatening and debilitating illnesses with a prolonged recovery time. Chrysaora spp. have been known to cause a diverse spectrum of manifestations. We report the case of a 7-year-old boy with jellyfish envenomation presenting with cyanosis, seizures and hypertension not previously described in association with the Chrysaora spp.

Most of the children present with lymphadenopathy. A rare subset of children do present with constellation of atypical symptoms as paraneoplastic syndromes. We hereby present an 11-year-old boy with classical Hodgkin’s Lymphoma associated with Alopecia areata and demyelination as paraneoplastic manifestations. Both these paraneoplastic manifestations improved after initiating chemotherapy (ABVD regimen). A high index of suspicion for underlying malignancy would help clinicians in clinching an early diagnosis and would avert the associated complications.

Here we report a case of a 8-year-old boy diagnosed with B cell acute lymphoblastic leukemia during chemotherapy, developed multiple vesicular erythematous cutaneous lesions in the venepuncture area of the left forearm, and histopathology was suggestive of Aspergillosis.

However, limited evidence is available addressing its risks and benefits. To study the efficacy and safety of HHHFNC in comparison to nCPAP for the facilitation of extubation in preterm neonates (born at 27-34 weeks of gestation) with respiratory distress.Methods: A prospective observational study was conducted, where 64 neonates were assigned either to nCPAP (n=34) or HHHFNC (n=30) groups post-extubation. The primary outcome was treatment failure (defined by pre-specified criteria) requiring a higher modality of respiratory support within 72 hours after extubation.Results: Treatment failure was seen in 36.7% of neonates assigned to the HHHFNC group compared to 14.7% in the nCPAP group (P=0.043). The fever is intermittent and high grade, associated with chills and sweating, with recurrences every 2 to 3 weeks. She also reports left flank and abdominal pain, with some associated distention. She feels full easily during meals and often wakes up from sleep due to pain. There is no pedal edema, breathlessness, decreased urination, or facial puffiness. She feels easily fatigued and is described as getting more “white” by the parents. Her family and developmental histories are normal. Details of previous evaluation and care are not available.

Isolated splenic abscess is rare in children, and occurs predominantly in the immunocompromised, ... more Isolated splenic abscess is rare in children, and occurs predominantly in the immunocompromised, with high rates of mortality and morbidity. We reviewed seven cases of splenic abscess managed at our centre. Six out of seven children (aged 7 months - 15 years, four male and three female) had multiple abscesses. Fever, abdominal pain, hepatosplenomegaly and leukocytosis were the predominant clinical manifestations. Burkholderia pseudomallei and coagulase-negative Staphylococcus aureus were isolated as the causative agents in two children. Five children were successfully managed conservatively, and two with ultrasonography-guided aspiration. Early institution of parenteral antibiotics and ultrasonography-/computed tomography-guided aspiration can help in preventing splenectomy in such children.

Introduction: Acute kidney injury/impairment (AKI) includes the entire spectrum of the syndrome f... more Introduction: Acute kidney injury/impairment (AKI) includes the entire spectrum of the syndrome from minor changes in markers of renal function to the requirement of renal replacement therapy. Objective: To study the incidence, clinical profile and outcome of AKI in the paediatric intensive care unit (PICU) of a tertiary care centre from South India. Method: A prospective observational study was conducted in a paediatric tertiary health care centre from South India. All children between 1 month and 18 years of age, admitted to the PICU from October 2015 to April 2016 were included in the study. Children with known pre-existing renal disease, children who underwent renal replacement therapy before admission and children admitted for postoperative care and who were discharged within 48 hours or against medical advice, were excluded from the study. Results: A total of 310 children was admitted to the PICU during the study period and 262 were considered for the study after excluding 48 cases. Male-female ratio in the study group was 1.12:1. At admission, AKI was present in 8% (21 out of 262), at 48 hours in 7.9% (20 out of 252, 10 children expired before 48hrs). Overall, pRIFLE criteria were observed in 31 (11.8%) out of 262 children during the hospital stay. The overall mortality rate of the study group was 9.2%. The length of PICU and hospital stay was significantly longer in children with AKI when compared to children without AKI (p-values 0.003 and <0.001 respectively). Most common aetiology implicated in the present study was sepsis (64.5%).

An effective and accurate biochemical marker is needed which can support or exclude the diagnosis of sepsis. Objective: To study the platelet indices in children with sepsis and their relation to the outcome Method: A prospective observational study was conducted in a tertiary care centre from southern India from October 2015 to July 2017. Children aged 1 month to 18 years of age with sepsis, as per the definition of International Paediatric Sepsis Consensus Conference 2005, were included in the study. Children with no infection were taken as controls. Children with haematological malignancies, known platelet disorders, haemolytic anaemia and children with a history of recent blood transfusions or splenectomy were excluded from the study. History and physical examination findings were noted in a pre-designed proforma. Platelet indices were measured using an automated blood counter, Beckman Coulter LH780 series. Counts done at the time of admission and prior to discharge or death were taken for analysis. Mean platelet volume (MPV), platelet distribution width (PDW) and plateletcrit (PCT) values were compared with standard normal values. Severity of thrombocytopenia was classified as mild (50,000-150,000/cu mm), moderate (20,000-50,000/cu mm) and severe (<20,000 or <50,000/cu mm with clinical bleeding) for the study purpose. Mean and median were used for data with normal and nonnormal distribution. Mann-Whitney was used for analysis of continuous variables. Chi-square was used for categorical variables. Analysis of mean was done using independent t-test. Data was analysed using SPSS version 21. Study protocol was approved by Institutional Ethics Committee
