Dr Dyer is a psycho-pharmacologist who conductes work related to the acute and chronic effects of drugs of dependence. He has a broad range of skills in psychological and pharmacological methodologies, workforce development, tertiary education, staff and student management, strategic and business planning, and clinical and political consultancy. Dr Dyer has been conducting research into the efficacy of point-of-collection oral fluid drug tests, the relationship between oral fluid drug concentration and psycho behavioural and cognitive impairment, and the utility of oral fluid for therapeutic drug monitoring within drug dependence treatment. Parallel research areas include the diagnosis and management of mental health problems and neuro-cognitive impairment among drug dependent individuals; the assessment and management of methamphetamine withdrawal and dependence; and the relationship between drug use and cognitive functioning, including the areas of drugs and driving and drug impairment in the workplace.

Effective dissemination from researchers to clinicians can improve outcomes for people using subs... more Effective dissemination from researchers to clinicians can improve outcomes for people using substance misuse services by providing the knowledge and skills necessary to deliver best practice. The internet has the potential to facilitate quick, accurate and affordable learning on a large scale. However, the quality of online resources for substance misuse worker training is rarely evaluated. Aim: To review the available literature on the learning outcomes, qualitative descriptions and costs of online learning. Methods: The literature on online learning, staff training and substance misuse were reviewed following PRISMA guidelines. Findings: Sixteen articles were identified with large variation in study quality and design. Descriptions of online interventions were insufficient for replication or comparison. Good quality online training should meet the needs of substance misuse workers whilst acknowledging that these needs will differ according to worker and context. Conclusions: Published research into online learning for the substance misuse workforce should be sufficient in detail to enable replication and direct comparison. More qualitative research about the needs and preferences of the workforce using online learning would fill a notable gap in the literature.

A systematic review of the relationship between staff perceptions of organizational readiness to change and the process of innovation adoption in substance misuse treatment programs

Translating innovation, such as contemporary research evidence, into policy and practice is a cha... more Translating innovation, such as contemporary research evidence, into policy and practice is a challenge, not just in substance misuse treatment programs, but across all spheres of healthcare. Organizational readiness to change (ORC) has been described as a fundamental concept, and an important determinant of the process of innovation adoption. The aim of this review was to describe the relationship between staff perceptions of ORC and the process of innovation adoption: exposure, adoption, implementation and integration into practice, in substance misuse treatment programs. This systematic review was conducted in accordance with PRISMA guidelines and fourteen papers were identified as being eligible for inclusion. This review was designed to include all constructs of ORC, but only one tool was used in all of the included papers. Despite this, the heterogeneity of studies in this review made a direct comparison of ORC related variables challenging. None of the included papers clearly related to one stage of the process of innovation adoption, and all of the included papers related to the early stages of the process. Only one paper attempted to measure the sustained integration of an innovation into practice. Overall , the papers were assessed as being low in terms of evidential hierarchy and the quality of the papers was assessed as being on average fair. ORC measurements provide us with a measure of organizational functioning which can be important in terms of predicting how successfully new innovations are adopted. Motivation for change was high in programs where staff identified more program deficits and these staff could also identify more specific needs, but were less likely to have exposure to new innovations. Better program resources and specific staff attributes, increase the likely hood of successful innovation adoption. A good organizational climate is potentially the strongest predictor for the adoption of new practices. It may be beneficial to measure ORC in conjunction with other variables such as program size, location, staff workloads and financial resources. More evidence is required on how to address ORC deficits once they are identified in practice.

Organizational readiness to change (ORC) is a fundamental concept within the field of organizatio... more Organizational readiness to change (ORC) is a fundamental concept within the field of organizational 10 development and has been operationalized in substance misuse treatment. The aim of this review was to describe the relationship between staff perceptions of ORC and outcomes in substance misuse treatment services. This systematic review adhered to PRISMA guidelines. Eight papers were identified for inclusion. All papers utilized the same tool or a modified version of the same tool to measure ORC. Four of these papers used the same tool to measure client outcomes. The quality of the included papers 15 was fair, and the level of evidence was low in respect of evidential hierarchy. ORC is predictive how effectively programs can deliver treatment. Positive ORC measurements relate to better staff/client relationships, reduced substance misuse, and better overall treatment engagement. Effectively resourced programs are more likely to have a more favorable ORC assessment, but resources are not the only determinant. Staff dynamics are important, and having a good organizational climate is of 20 particular importance. There is limited evidence available on how to address ORC deficits. Understanding which organizational attributes are more effective than others could provide important information for policy makers and planners, but more evidence is required.
