However, for more than 60 years, antibiotics have been regarded as the panacea, as long as they are used wisely and timely. Antibiotic resistance has escalated in recent years, resulting in an accelerating global health security emergency, that is rapidly outpacing available treatment options. In January 2014, the new mandatory health insurance scheme (JKN) was introduced, whose treatments must comply with National Formulary (FORNAS) policy. We aimed to systematically review the prevalence of antibiotic resistance to FORNAS policy's preferential treatments in adult septic patients who had been in the non-surgical wards. Based on an overall view, 76 out of 90 kinds of antibiotics which had undergone antibiotic susceptibility test (AST) had alarming resistance rate and preferential antibiotics in the current JKN scheme may have become ineffective.

Peritoneal tuberculosis may lead to delayed diagnosis because of the nonspecific features such as... more Peritoneal tuberculosis may lead to delayed diagnosis because of the nonspecific features such as fever, abdominal distension, abdominal tenderness, ascites, and weight loss. Here, wereported a case of prolonged fever and abdominal pain which was due to peritoneal tuberculosis. Initial examinations including acomplete blood test and serologic tests did not lead to the diagnosis. A final diagnosis was made by abdominal CT-scan and laparoscopy combined with histopathological studies. Antituberculous medications provided a good clinical response in this patient.

The Sumatra Fault System is elongated across the Sumatra Island, Indonesia, Southeast Asia includ... more The Sumatra Fault System is elongated across the Sumatra Island, Indonesia, Southeast Asia including the central part of West Sumatra, Indonesia, Southeast Asia. The Sumatra Fault and subsurface structure on the Central Part of West Sumatra had been analyzed using gravity method. Bouguer anomaly data were obtained from GRDC (Geological Research and Development Centre) maps, Bandung, Indonesia (i.e. without terrain correction). In this study, terrain correction had been applied to these Bouguer data. Bouguer anomaly in a horizontal plane at 3000 meters high and equivalent depth of mass point 7000 meters were obtained using Dampney Method. Residual and regional anomalies were separated using upward continuation method at 8000 meters high. The result of the SVD on residual anomaly shows two negative anomalies on northwest-southeast. The zero miligal per meter square quantity coincides remarkably well with trace faults which is a part of the Sumatra Fault System. Two negative anomalies are located around the Sianok Segment and Sumani Segment.

Severe clinical manifestation can be produced by this organism. It varies according to immune status, and subtype of this organism. Unfortunately, we found an immunocompetent patient with chronic diarrhea caused by this organism. A 38year old male was admitted to Adam Malik General Hospital because of watery diarrhea since four days ago. Administration of fluid replacement was done to this patient, but the frequency of diarrhea did not decrease. Loperamide as anti-spasmodic was also given in each episode of diarrhea. Surprisingly, fecal smear examination revealed that this patient positive for Blastocystis hominis and Cryptosporidium sp. Thus, diarrhea was resolved for four days without giving any anti-parasitic drugs to the patient.

Research on red color melinjo peels containing anthocyanin active substance has been studied sinc... more Research on red color melinjo peels containing anthocyanin active substance has been studied since 2011, starting from anthocyanin extraction stage (maceration) by using both polar and non-polar solvents, a variation of citric acid addition and stability of anthocyanin extract storage and application of anthocyanin for food and beverage. However, research on the identification of anthocyanin chemical structures and other active substances of red color melinjo peels is also necessary. The purpose of this study was to identify the structure of anthocyanin and other active substances from red color melinjo peels with FTIR and LC-MS analysis. The results obtained: from FTIR analysis shown wavelength, among others: 3425 cm-1, 1725 cm-1, 1632 cm-1 and 1219 cm-1, it shows that inside the red color melinjo peels contained OH function group, C=O, C=C, and CO. Research on red dye foods containing anthocyanin derived from red color melinjo peels has been do... more Research on red dye foods containing anthocyanin derived from red color melinjo peels has been done since 2011 in a laboratory scale. However, to increase anthocyanin production to a larger scale (domestic industry) requires further research. The purpose of this study was to determine the effect of scale-up treatment from the amount of red color melinjo peels on the acquisition of anthocyanin extract. The study was conducted by the maceration method for 3 days, using 96% ethanol solvent with the addition of 3% citric acid. Comparison (m / v) of red color melinjo peels/ethanol was 1: 5 with red color melinjo peels mass of 100 g, 200 g, 300 g, 400 g, respectively. The results showed that the anthocyanin concentration from a red color melinjo peels mass of 100 g, 200 g, 300 g, 400 g was 2.1709 mg /g, 2.3046 mg/g, 2.3044 mg/g, 2.0039 mg/g with a density of 1.1596 g/ ml, 1.1604 g/ml, 1.1604 g/ml and 1.1596 g/ml, respectively.

Cancer is one of the biggest causes of death in the world, including in Indonesia and there is no... more Cancer is one of the biggest causes of death in the world, including in Indonesia and there is not yet found specific drugs to cure cancer. Treatment using drugs is one of therapies that can be used to treat cancer patients, but at the same time, it has side effect which is affecting healthy cells, resulting problem such as muscle, kidney, or fertility problems. Therefore, virotherapy is introduced as alternative cancer therapy by utilizing oncolytic virus as a weapon to damage cancer cells without harming healthy cells. Nonlinear model of cancer treatment using combination of drug therapy and virotherapy can describe the dynamics of cancer treatment phenomenon. From the model, drug therapy and virotherapy will be assigned as control that will minimize the side effect of therapy. Here we discuss Linear Parameter Varying (LPV) as an approach to determine the control input with a set of parameters that vary in specific interval. Numerical simulation shows that dynamical behaviour of LPV model is similar with nonlinear model. From LPV controller, it can be obtained the dosage of drugs and virotherapy to minimize the growth of cancer cells. Furthermore, LPV model can interprets less complex and is more useful since it relates to the desired trajectory and can generates the unseen time variation.

Mangrove forests are wetland resources in coastal areas that important role in supporting of life... more Mangrove forests are wetland resources in coastal areas that important role in supporting of life. However, in recent decades, many mangrove forests have been degraded or lost, mainly due to the conversion of mangroves into ponds. Mangrove restoration activities on ex-pond lands have begun in recent years on the eastern coast of North Sumatra. This study aimed to measure the recovery of mangrove species through restoration activities on ex-pond lands and to know the composition of the restored mangrove forests at the eastern coast of North Sumatra. A total of 30 sample plots, each plot size of 10m by 10m, have been established at each age of mangrove stands (5-9 years) and secondary forests. All trees with the diameter (DBH) ≥ 2 cm in the sample plot were identified and measured the DBH. Overall, there were 14 mangrove species in all sample plots, equivalent to 42.42% of all true mangrove plants found in Indonesia. This result indicated that mangrove restoration has succeeded in restoring mangrove species diversity. Therefore, the mangrove restoration on ex-pond land needs to be promoted in an effort to restore various mangrove functions for community and coastal resources resilience.

Bantargebang integrated waste treatment (TPST Bantargebang) is the first landfill completed with energy recovery facility, landfill gas technology. TPST Bantargebang power plant established in 2010 and supplied electricity to grid. The electrical energy supplied tend to decrease and only reaches 2.4 GWh in 2017. Energy potential is important to evaluated for knowing the performance of TPST Bantargebang power plant. By using Modified Triangular Method, obtained the results that the electricity energy potential equal to 4.5 GWh. The calculation also estimate the emission reduction potential about 4325.88 tCO2/year, and economic potential from sales of electricity about 3.7 billion rupiah.

Mesoscale Convective Complex (MCC) is an atmospheric disturbance in the form of convective cloud ... more Mesoscale Convective Complex (MCC) is an atmospheric disturbance in the form of convective cloud system which has coverage area more than 100,000 km 2 and a cloud core reaching an area of 50,000 km 2 and able to survive for at least 6 hours. The existence of this system can produce several bad weather conditions for the covered area, can even lead to disasters such as floods and landslides. This paper aimed to identify the occurrence of MCC during active and break Asian winter monsoon in December, January and February 2016. The results showed that when the Asian Winter monsoon was being active in Indonesian territory, the number of MCC formed was higher than that in the break period of Asian monsoon. These results indicated that Asian winter monsoon could trigger the formation of MCC.

Employee engagement (EE) devided in two dimension: job engagement (JE), and organization engagement (OE). JE and OE were tested as mediating variables. Data were collected through questiionnaires to 164 employees of banking industry in Bandung and Jakarta. The results of this research showed that the two dimensions of EE have different meaning. Organizational support and organization commitment significantly acted as antecedents of JE and OE. Meanwhile, job characteristics, reward, management support and procedural justice have not found as antecedents of JE and OE. Employee engagement found correlated with organizational commitment, intention to leave, organizational citizenship behavior organization (OCBO), organizational citizenship behavior individual (OCBI), and job satisfaction. As the conclution, employee engagement has found partially mediating factors of antecedents and consequences of employee engagement.

The objective of this research is to examine the effect of work stress on performance, motivation... more The objective of this research is to examine the effect of work stress on performance, motivation on performance, and work stress and motivation on performance. The research was conducted at Lembaga Pemasyarakatan Klas IIA Anak Pria Tangerang with total sample of 97 correctional officers. The results showed that: (1) Work stress negatively effect work performance of correctional officer and not significant. This result supported by correlation test which r value is -0.011, with b’s value = -0.027 and R square = 0.00019. (2) Work motivation positively effect on work performance of correctional officer which r value of 0.359, with b’s value = 0.655 and R square = 0.129. Finally researcher provides the important managerial implications useful for correctional officer related to work stress, motivation, and performance that will lead to work performance of correctional officer.

Data of child tuberculosis, clinical visits and those who were treated in the three hospitals, vi... more Data of child tuberculosis, clinical visits and those who were treated in the three hospitals, visits to the 6 public health centers (Puskesmas) and figures of BCG immunization in the North Sumatra Province were presented. Although the tuberculin index in children in Medan (1965) was lower than that in Java (BCG Mass Campaign, 1954-1964), primary tuberculosis patients treated in the General Hospital (RSUPP) constituted 1.14% of the total patients admitted to the hospital in 1963-1972, whereas in the "PNP I X " hospital it was 0.84%. From a survey o f families (1962-1969) it revealed that infection in the families was ± 50%. While 76.1% of the meningitis patients were under the age of 4 years. From a survey of 6 public health centers we found through a Pilot Project that 1.02% patients had a positive sputum. The above mentioned figures gave the indication that BCG immunization had to be performed in children under the age of 4 years. However, the figures of the BCG campaign (1970-1973) at the above mentioned Pilot Project revealed that coverage in the under 5-years-old age was 20.5% , whereas in the 5-14 years age group it was 49.2%. In practice the realization of massal BCG immunization was certainly the simplest with school age children who were already grouped and registered; nevertheless, partly of them h a i already got infected and had been released from tuberculous meningitis complication. BCG immunization as mentioned above (probably it was also given m other areas in Indonesia) did obviously not reach its right target.

This paper presents a reduced order model problem using reciprocal transformation and balanced tr... more This paper presents a reduced order model problem using reciprocal transformation and balanced truncation followed by low order controller design of infinite dimensional systems. The class of systems considered is that of an exponentially stable state linear systems (, ,) A B C , where operator A has a bounded inverse, and the operator B and C are of finite-rank and bounded. We can connect the system (, ,) A B C with its reciprocal system via the solutions of the Lyapunov equations. The realization of the reciprocal system is reduced by balanced truncation. This result is further translated using reciprocal transformation as the reduced-order model for the systems (, ,) A B C. Then the low order controller is designed based on the reduced order model. The numerical examples are studied using simulations of Euler-Bernoulli beam to show the closed-loop performance.

This paper is concerned with the application of reduced-order controller to a flexible structure ... more This paper is concerned with the application of reduced-order controller to a flexible structure hav ing the transverse-torsional coupled vibration modes. The dynamical model of the structure is first presented and then the robust controller for the structure is designed by using the H ∞-control theory. Furthermore, the order of the controller is reduced by using the combination of the alternating-projection method and the weighted-balanced realization. The performance of the reduced-order controller is examined through simu lation and experiment. To verify its robustness and validation in the transverse-torsional coupled vibration condition, the lumped mass is added on the right side at the 3rd story. From these results, it is made clear that the reduced-order controller obtained by the proposed method is effective in reducing the vibration of the structure, where the order of the controller can be reduced to 8th order from 30th order.
