Childhood obesity is associated with medical and psychological comorbidities, and interventions t... more Childhood obesity is associated with medical and psychological comorbidities, and interventions targeting overeating could be pragmatic and have a significant impact on weight. Calorically dense foods are easily available, variable, and tasty which allows for effective opportunities to learn to associate behaviors and cues in the environment with food through fundamental conditioning processes, resulting in measurable psychological and physiological food cue reactivity in vulnerable children. Basic research suggests that initial learning is difficult to erase, and that it is vulnerable to a number of phenomena that will allow the original learning to re-emerge after it is suppressed or replaced. These processes may help explain why it may be difficult to change food cue reactivity and overeating over the long term. Extinction theory may be used to develop effective cue-exposure treatments to decrease food cue reactivity through inhibitory learning, although these processes are complex and require an integral understanding of the theory and individual differences. Additionally, learning theory can be used to develop other interventions that may prove to be useful. Through an integration of learning theory, basic and translational research, it may be possible to develop interventions that can decrease the urges to overeat, and improve the weight status of children.

This article describes the development, implementation, and results of a focused intervention to reduce SSB sales within a hospital setting. After a formative research process, Rethink Your Drink was launched at a children's hospital in San Diego. The initiative consisted of an educational intervention using the stoplight system to categorize beverages as red, yellow, or green based on sugar content. Beverage sales data were collected for 3 months prior, during the 12-month intervention, and for 4 months after the intervention ended. Monthly red beverage sales decreased from an average of 56% during baseline to 32% at the end of the data collection period (p < .001). Monthly green beverage sales increased from an average of 12.2% during baseline to 38% at the end of the data collection period (p < .001). Sales revenue for all drinks remained constant. The objective of this study was to review the current eating disorders outcome literature after r... more The objective of this study was to review the current eating disorders outcome literature after residential or partial hospitalization programme (PHP) treatment. Articles were identified through a systematic search of PubMed and PsycINFO. Twenty-two PHP and six residential treatment studies reported response at discharge and tended to find improvement. Fewer studies (nine PHP and three residential) reported outcome at some interval after discharge from treatment. These tended to find sustained improvement. A substantial proportion of patients were lost to follow-up, particularly for residential treatment. Only two follow-up studies used controlled trials; both showed efficacy for PHP compared with inpatient treatment with regard to maintaining symptom remission. Improvement at discharge may not predict long-term outcome. Long-term follow-up studies were confounded by high dropout rates. Characteristics of a Favorable Weight Status Change From Adolescence to Young Adulthood

To explore 10-year longitudinal predictors (personal, psychological, behavioral, and socioenviron... more To explore 10-year longitudinal predictors (personal, psychological, behavioral, and socioenvironmental) of exiting obesity from adolescence to young adulthood. Data were collected from a population-based cohort of adolescents (n = 2,287) attending middle/high schools in Minneapolis-St. Paul in 1998-1999 (mean age = 14.9) and again in 2008-2009 (mean age = 25.3) participating in Project Eating and Activity Among Teens and Young Adults. Self-reported height and weight were used to calculate weight status change between adolescence and young adulthood, among participants with obesity at baseline (n = 175). Questionnaires assessed personal, psychological, behavioral, and socioenvironmental factors hypothesized to play a role in obesity. Modified Poisson regressions estimated adjusted relative risks (RRs) for exiting obesity as a function of each baseline and 10-year change in predictor, controlling for relevant covariates. Thirty-two percent of adolescents exited obesity in young adulthood. Reductions in fast food intake (RR = .73, 95% confidence interval [CI] = .61-.87) and screen time (RR = .98, 95% CI = .96-.99), and increases in fruit/vegetable intake (RR = 1.06, 95% CI 1.01-1.12), moderate-to-vigorous physical activity (RR = 1.06, 95% CI = 1.00-1.12), home fruit/vegetable availability (RR = 1.58, 95% CI = 1.19-2.09), family meals (RR = 1.12, 95% CI = 1.03-1.22), and serving vegetables at dinner (RR = 1.45, 95% CI = 1.10-1.92) were associated with exiting obesity. Not dieting as an adolescent and improvements in body satisfaction, depressive symptoms, self-esteem, and weight teasing were also associated with exiting obesity. Promoting healthy eating and activity, and improving the healthfulness of home food environments may be promising intervention targets for promoting healthier weights in adolescents and young adults with obesity. Addressing dieting behavior and the psychosocial health of adolescents with obesity may also be needed throughout the transition to young adulthood.

Warm Parenting Associated with Decreasing or Stable Child BMI during Treatment

While authoritative parenting, which includes high levels of warmth and behavioral control, has b... more While authoritative parenting, which includes high levels of warmth and behavioral control, has been associated with lower risk of obesity, little is known about how general parenting impacts child weight loss during treatment. Our goal was to examine the relationship between several general parenting dimensions and &#39;decreasing /stable&#39; child BMI during a 16-week family-based behavioral weight control program. Forty-four overweight parent-child dyads (child age 8 to 12 years) enrolled in the program. Families were videotaped at baseline eating dinner in their home. Using the General Parenting Observational Scale (GPOS), meals were coded for several general parenting dimensions. Primary outcome was percent of children whose BMI &#39;decreased or stayed the same.&#39; Multivariable logistic regression was used to determine the relationship between general parenting and decreasing/stable child BMI. Forty families (91%) completed the program. Children had a mean BMI change of -0.40 (SD 1.57), which corresponds to a -0.15 (SD 0.20) change in BMI z-score (BMI-Z); 75% of children had decreasing/stable BMI. In the unadjusted models, lower parent BMI, higher parent education, and higher levels of parental warmth were significantly associated with decreasing/stable child BMI. In the multivariable model, only higher level of warmth was associated with increased odds of decreasing/stable child BMI (OR = 1.28; 95% CI, 1.01, 1.62). Baseline parental warmth may influence a child&#39;s ability to lower/maintain BMI during a standard family-based behavioral weight control program. Efforts to increase parent displays of warmth and emotional support towards their overweight child may help to increase the likelihood of treatment success.

Therefore, we examined these relationships in 117 treatment-seeking overweight and obese children (10.40 ± 1.35 years; 53% female; 52% Caucasian; BMI-z: 2.06 ± .39). Children consumed a dinner meal, completed an Eating in the Absence of Hunger (EAH) free access paradigm (total EAH intake = EAH%-total; sweet food intake = EAH%-sweet), and completed the Child Report of Parent Behavior Inventory. Parents completed the Child Feeding Questionnaire. Child EAH%-total and EAH%-sweet were positively associated with dinner consumption (p's < .01). Girls had significantly higher EAH%-total compared to boys (p < .05). Secondary data analysis of a randomized controlled trial conducted at two university medical centers with 227 overweight adults with diabetes. Logistic and standard regression analyses examined associations between emotional eating change and weight loss success (i.e., weight loss of ≥7 % of body weight and decrease in BMI). After 6 months of intervention, decreased emotional eating was associated with greater odds of weight loss success (p = .05). The odds of weight loss success for subjects with decreased emotional eating at 12 months were 1.70 times higher than for subjects with increased emotional eating. Objective: To assess whether weight-related teasing is associated with weight control behaviors, ... more Objective: To assess whether weight-related teasing is associated with weight control behaviors, disordered eating thoughts and behaviors, and psychological comorbidities in overweight adolescents. Methods and Procedures: A sample of 46 male and 84 female adolescents completed a survey assessing teasing frequency, sources of teasing (peers and family), weight control behaviors, disordered eating thoughts and behaviors, depression, anxiety, anger, and self-esteem. Results: Frequent teasing by both family and peers was associated with greater disordered eating thoughts and behaviors, depression, anxiety, anger, and decreased self-esteem. The more that adolescents were bothered by peer and family teasing, the more often they reported a greater value on thinness, higher levels of anxiety, lower self-esteem, and their self-assessment was influenced by their weight and shape. Higher levels of teasing frequency and being bothered by teasing were related to greater odds of adolescents endorsing severe levels of binge eating behaviors and depressive symptomolgy. Discussion: Overweight adolescents teased about their weight are at risk for disordered eating thoughts and behaviors and psychological morbidities. Health-care providers should involve parents and youth in discussing teasing concerns. Furthermore, parents, schools, and communities should consider enacting policies to decrease or prohibit teasing. Future research is needed to further explore relationships between teasing and psychological functioning.

There is a continued need to improve upon the efficacy and availability of treatments for anorexi... more There is a continued need to improve upon the efficacy and availability of treatments for anorexia nervosa. Family-based therapy for anorexia nervosa demonstrates strong empirical evidence; however, trained treatment providers are limited and a subsample of participants receiving the treatment fail to respond. The intensive family treatment program is a brief, time-limited, multi-family program that trains families of adolescents with eating disorders to oversee their adolescents' recovery at home by providing psychoeducation, skills training, and immersive practice over the course of a 5-day period. This article provide a description of the program by summarizing underlying theoretical principles and key therapeutic components.

Background: The causes of recent increases in the prevalence of overweight in the United States a... more Background: The causes of recent increases in the prevalence of overweight in the United States are perplexing because national survey data do not show clear patterns of change in energy intake or expenditure. Objective: Ecologic data regarding the quantities and types of foods and nutrients available in the United States between 1970 and 1998 were reviewed to provide an alternative perspective on trends in energy intake. Design: Literature searches in agriculture, business, and medical library databases were conducted to identify data regarding the quantities and types of foods and nutrients available in the United States between 1970 and 1998. Results: Per capita energy availability estimates from the US Department of Agriculture, US Food Supply Series, indicate that energy availability increased by 15% between 1970 and 1994. Data regarding trends in food purchasing and preparation suggest that Americans are eating more meals outside the home, relying more heavily on convenience foods, and consuming larger food portions. Americans appear, in some cases, to be selecting lower-fat foods over higher-fat alternatives. In addition, availability and sales of reduced-energy and reduced-fat products have increased. Conclusion: Consistent with trends in overweight, most of the ecologic data reviewed suggest that energy intake has increased over the past several decades and is likely a major contributor to increases in average body weight.

Family-based weight loss treatment (FBT) is considered the gold-standard treatment for childhood obesity, but FBT is both staff and cost intensive. Therefore, we developed the FRESH (Family, Responsibility, Education, Support, & Health) study to evaluate the effectiveness of intervening with parents, without child involvement, to facilitate and improve the child's weight status. Targeting parents directly in the treatment of childhood obesity could be a promising approach that is developmentally appropriate for grade-school age children, highly scalable, and may be more cost effective to administer. Family therapy approaches have generated impressive empirical evidence in the treatment of adoles... more Family therapy approaches have generated impressive empirical evidence in the treatment of adolescent eating disorders (EDs). However, the paucity of specialist treatment providers limits treatment uptake; therefore, our group developed the intensive family therapy (IFT)a 5-day treatment based on the principles of family-based therapy for EDs. We retrospectively examined the long-term efficacy of IFT in both single-family (S-IFT) and multi-family (M-IFT) settings evaluating 74 eating disordered adolescents who underwent IFT at the University of California, San Diego, between 2006 and 2013. Full remission was defined as normal weight (≥95% of expected for sex, age, and height), Eating Disorder Examination Questionnaire (EDE-Q) global score within 1 SD of norms, and absence of binge-purging behaviours. Partial remission was defined as weight ≥85% of expected or ≥95% but with elevated EDE-Q global score and presence of binge-purging symptoms (<1/week). Over a mean follow-up period of 30 months, 87.8% of participants achieved either full (60.8%) or partial remission (27%), while 12.2% reported a poor outcome, with both S-IFT and M-IFT showing comparable outcomes. Short-term, intensive treatments may be cost-effective and clinically useful where access to regular specialist treatment is limited.

The relationship between parent feeding styles and general parenting with loss of control eating in treatment-seeking overweight and obese children

To examine differences in parent feeding behaviors and general parenting of overweight children w... more To examine differences in parent feeding behaviors and general parenting of overweight children with and without loss of control (LOC) eating. One-hundred-and-eighteen overweight and obese children (10.40 ± 1.35 years; 53% female; 52% Caucasian; BMI-z: 2.06 ± 0.39) and their parents (42.42 ± 6.20 years; 91% female; 70% Caucasian; BMI: 31.74 ± 6.96 kg/m(2) ) were seen at a baseline assessment visit for a behavioral intervention that targeted overeating. The Eating Disorder Examination, adapted for children (ChEDE) was administered to assess for LOC eating. Parents completed the Parental Feeding Styles Questionnaire (PFSQ) and the Child Feeding Questionnaire (CFQ) to assess parent feeding styles and behaviors. Children also completed a self-report measure of general parenting (Child Report of Parent Behavior Inventory, CRPBI-30). Forty-three children (36.40%) reported at least one LOC eating episode in the month prior to assessment. Parents who reported greater restriction and higher levels of pressure to eat were more likely to have children that reported LOC eating (ps &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt; 0.05). Parents who utilized more instrumental feeding and prompting/encouragement to eat techniques were less likely to have children that reported LOC eating (ps &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt; 0.05). Child-reported parenting behaviors were unrelated to child LOC eating (ps &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;gt; 0.05). Parent feeding styles and behaviors appear to be differentially and uniquely related to LOC eating in treatment-seeking overweight and obese children. Future research is needed to determine if implementing interventions that target parent feeding behaviors may reduce LOC eating, prevent full-syndrome eating disorders, and reduce weight gain in youth. © 2015 Wiley Periodicals, Inc. Unfortunately, cardiac rehabilitation exercise programs have high drop out rates. Several studies with noncardiac patients indicate that exercise-related strategies engaged in during exercise play an important role in exercise performance and adherence. We assessed exercise-related cognitions utilized by a group of cardiac rehabilitation patients who were adherent to a structured exercise program. Thirty-six subjects provided information on their cardiac history and exercise patterns and reported on the thoughts that they might have when exercising in situations depicted in 11 vignettes. Responses, rated using a reliable coding scheme, revealed that the majority of thoughts involved evaluating progress relative to goals and engaging in motivational self-talk. Other strategies reported included using cognitive distraction, thinking about body shape and arousal, making comparisons between self and others, and engaging in social distraction and interaction. Findings suggest that the exercise adherent subjects in this study use specific cognitive and behavioral strategies. Future studies comparing exercise thoughts of both adherent and nonadherent persons and prospective evaluation of their relationships to exercise adherence appear warranted.
