Differences between Current and Past Self-Injurers: How and Why Do People Stop?

Using the Experiential Avoidance Model (EAM), we examined whether characteristics of self-injurers remain in people who have successfully ceased self-injury and what, by implication, might be targeted to improve therapeutic efficacy. An online cross-sectional survey of 215 1st year university students. Past self-injurers (34) scored significantly better on subscales of the General Health Questionnaire, as well as Distress Tolerance, Experiential Avoidance and Self-blame compared to Current self-injurers (29) Conclusion: The Experiential Avoidance Model is a useful basis for understanding self-injury, and informing therapeutic approaches. Reducing Anxiety, and developing Tolerance and Positive Emotional Intensity may be keys to ceasing self-injury.

Longitudinal Analysis of Adolescent NSSI: The Role of Intrapersonal and Interpersonal Factors

Non-suicidal self-injury (NSSI) occurs in approximately 10 % of adolescents. To establish effecti... more Non-suicidal self-injury (NSSI) occurs in approximately 10 % of adolescents. To establish effective prevention and intervention initiatives, it is important to understand onset, maintenance and cessation of NSSI. We explored whether the relationships between interpersonal factors (i.e. attachment, social support) and NSSI were mediated by intrapersonal factors (i.e. emotion regulation, self-esteem, self-efficacy). Participants were 1973 students (1414 female and 559 male) aged between 12 and 18 years (M =13.89, SD =0.97) recruited from 40 Australian high schools. Participants completed a questionnaire at two time-points with a 12-month interval. At baseline, 8.3 % of adolescents engaged in NSSI, increasing to 11.9 % at follow-up. Family support was most salient in onset, maintenance and cessation of NSSI. Attachment anxiety was related to NSSI onset. Of the intrapersonal variables, self-esteem and self-efficacy were significant in predicting onset of NSSI. Self-esteem, self-efficacy and cognitive reappraisal mediated the relationship between attachment anxiety and NSSI onset. A combination of interpersonal and intrapersonal variables contributes to the onset, maintenance and cessation of NSSI in adolescence. Perceived family support appears to be an important safeguard against NSSI. Strategies targeting family functioning and teaching cognitive reappraisal techniques to adolescents may reduce the number engaging in NSSI.

A 20-week universal mental health promotion programme (the Aussie Optimism Program), based on cog... more A 20-week universal mental health promotion programme (the Aussie Optimism Program), based on cognitive-behavioural intervention procedures and targeting cognitive and social risk and protective factors, was delivered to 417 year eight students (mean age 13 years) from three secondary schools in Brisbane, Australia. Students were assessed with the Centre for Epidemiological Studies Depression Scale, Children's version, and the Strengths and Difficulties Questionnaire before and after completing the programme. Results indicated that the programme was most beneficial for students experiencing emotional and behavioural difficulties before commencement of the programme; however, the lack of a control group and other methodological limitations prevents conclusions being drawn about the efficacy of this programme. Nevertheless, the results raise the question of whether the costs of developing and maintaining a universal mental health promotion programme outweigh the benefits of helping only a small proportion of students.

A transdisciplinary approach to adolescent forensic mental health

To examine new strategies which may be implemented to address the significant mental health and s... more To examine new strategies which may be implemented to address the significant mental health and substance abuse problems of young people within the juvenile justice system. Wide-ranging literature review of mental health problems within the juvenile justice population is given, illustrating the high prevalence of mental health problems within this cohort of young people. Reference is made to the differing demographics and agendas of the American justice system compared to that found in Australia. It is suggested that new initiatives stemming from quality Australian studies are required in order to facilitate reform within adolescent forensic mental health. Psychiatrists need to be at the forefront of innovative policy delivery within the juvenile justice system. A transdisciplinary approach is required to meet the changing needs of young people within the juvenile justice system. Such a system of care recognizes that these young people and their families have multiple needs that cross traditional boundaries and a collaborative approach across agencies is essential at both the policy and practical level. Psychiatrists have an important role to play in the development of these services. A systemic process to address such needs is offered.

For people who self-injure, the internet can provide an anonymous environment where users feel sa... more For people who self-injure, the internet can provide an anonymous environment where users feel safe to divulge feelings and experiences which are not usually socially sanctioned. However, internet users can encourage each other to engage in self-destructive behaviour. This qualitative study aimed to simulate a typical search an adolescent might perform if he/she was looking for support and information about self-injury. The aim was to determine whether the resulting content was helpful or harmful. Seven queries -'cutting', 'self injury', 'self-injury', 'self harm', 'self-harm', 'self-mutilation', and 'self mutilation' -were entered into www.google.com and each link from the first webpage from each search was opened and judged for relevancy. Thirty-nine links were analysed and grouped into five categories: websites (n=5); webpages (n=11); book results (n=8); news results (n=11); and other (n=4). Examination of the quality and content of the links obtained from our search showed mainly recovery oriented, supportive information about self-injury. Despite this positive outcome, more specific searches such as 'pro-self-injury' would likely result in less encouraging results. Mental health professionals should be aware that their self-injuring clients have probably used the internet for support and information, and consider how this use might impact on therapy. Mental health professionals could use the results of this study to recommend quality internet sites to their clients.

Perceived Academic Performance as an Indicator of Risk of Attempted Suicide in Young Adolescents

This study investigated perceived academic performance and selfreported suicidal behavior in adol... more This study investigated perceived academic performance and selfreported suicidal behavior in adolescents (n = 2,596), mean age 13 years, from 27 South Australian high schools. Groups perceiving their academic performance as failing, below average, average and above average were significantly different on measures of self-esteem, locus of control, depressive symptoms, suicidal thoughts, plans, threats, deliberate self-injury, and suicide attempts. Multivariate logistic regression analyses revealed that failing academic performance (compared to above average) is associated with a five-fold increased likelihood of a suicide attempt, controlling for self-esteem, locus of control and depressive symptoms. Teachers should note that a student presenting with low self-esteem, depressed mood and perceptions of failure may be at increased risk for suicidal thoughts and behaviors, and need referral for clinical assessment.

Before and after viewing the video, parents (N = 112) of young people answered questionnaires measuring their (1) knowledge of suicidal signs, (2) responses to a young person showing suicidal signs, (3) attitude to suicide, and (4) intentionalitv toward suicidal people. After the video, parents&#39; knowledge of suicidal signs, responses toward a suicidal person, and intention to help improved. Their attitude was more rejecting of suicide. Knowledge and intentionality predicted response to suicidal statements, but no relationships existed between attitude and the other variables. Implications for youth suicide education programs are discussed.
