We see in 'Experiment in Terra' that Battlestars' have forcefield technology so presumably they use it to prevent that happening.
Undetermined, we only ever see 5 although the radio transmissions during the battle suggest there may be up to a dozen.
According to the Battlestar Galactica annual each ship carries 75 plus numerous shuttles. On Carrillon the Cylons refer to 250 warriors (pilots) being the ship's full complement but we also know that the Galactica recovered numerous Vipers from other destroyed Battlestars so she may be overloaded with fighters. Presumably the fleet can mass produce Vipers as Starbuck and Appollo regularly wreck theirs and have no problem finding replacements.
