One of the main villains, Darth Nihilus can scare to some. Mainly to his haunting voice because he isn't a human rather a spirit
One antagonist, Darth Sion, may be frightening to children. He has an intimidating aura due to his voice and his deformed, necrotic appearance.
There are no "shock" scenes, but some environments may be considered as grim or frightening. One example is the Peragus space station which is overrun with corpses and no signs of life, with various holographic and audio recordings depicting the crews' demise. Another is a Republic ship full of dead bodies, dim moody lighting, and invisible/stealthy enemies that constantly ambush the player and their squadmates.
KOTOR 2 has a rather grimy and unusually dark atmosphere for a Star Wars game. Some levels feel isolating and almost creepy, which may make some players on edge.
The game touches upon numerous discussions intended for mature players, such as war, life, morality etc.
