While there is mostly added dialogue, much of which is R- rated, it is no worse than the rest of the movie and actually adds more funniness to the movie.

Basically the Unrated version features several prolonged dialogue sequences that add up to a total difference of approximately 6 minutes. These extensions aren't that scandalous but for a fan of this movie this version is worth a look and features some more funny scenes. You can find a detailed comparison between both versions with pictures here.
The film is set when it was made, 2006. Look at McLovin's ID for reference.
It is never known for sure, but in a deleted scene it is revealed that Evan does in fact have a brother.
