Being close to my team I would usually identify this situation early. I would analyze if the situation is the entire team or if there are groups. From there I would have a private conversation with the project manager who is typically close to the team. I then would reach out to each of the team privately to discuss the projects progress, identify barriers, and actively listen. Depending on the severity, removing barriers the team had I would have a team meeting, recast the projects vision, review progress, review future tasks and who has what responsibility and celebrate milestones. Usually these steps would get the team back on track with even more momentum and motivation.
