I, ELVAH PELENIO, Head-Collection Department of the
BAYAN CREDIT CORPORATION, Tacloban City, under oath
state: THAT
On ____________________ I mailed by registered mail a
legible copy of the DEMAND LETTER with NOTICE OF
DISHONOR

________________________________________ in the Post Office of
Tacloban City in a sealed envelop with instruction to the
postmaster to return the mail to sender after ten (10) days if
undelivered, as evidenced by REGISTRY RECEIPT No.
____________ HERETO ATTACHED as proof of service, pursuant
to Sections 7 and 13 of Rule 13 of the 1997 Rules of Civil
Procedure. The registry return card shall be filed immediately
upon its receipt by the sender, or in lieu thereof, the
unclaimed letter together with the certified or sworn copy of
the notice given by the postmaster to the addressee.
