Affidavit of Missing Bank Fixed Deposit Receipt

Kolisetty Nethaji deposited Rs. 10,000 in a fixed deposit account with Sudha Co-op Urban Bank Ltd on June 7, 2011 that was set to mature on September 6, 2011. He has misplaced the original fixed deposit bond certificate while shifting residences and despite his best efforts, has been unable to locate it. He is now in need of a duplicate fixed deposit certificate in order to collect the maturity amount. He affirms that the information in the affidavit is true and correct to the best of his knowledge. He requests that the Branch Manager issue a duplicate certificate using their discretionary powers in the interest of justice.

BEFORE THE BRANCH MANAGER, SUDHA CO-OP. URBAN BANK LTD., SURYAPET AFFIDAVIT I, KOLISETTY NETHAJI, S/o.

Chandra Shekar, aged 39 years, Occ: Govt. Teacher, R/o. H.No. 1-4-249/102/B1/1, Balaji Nagar, Suryapet508213, Dist. Nalgonda do hereby solemnly affirm and state on oath as follows : 1. I am the deponent herein, hence I know the facts of this affidavit. 2. That I have deposited a Fixed Deposit under A/c. No. 5284FF on 07.06.2011 for an amount of Rs. 10,000/- with maturity date 06.09.2011 on my name, which is true and correct. 3. That the said original Fixed Deposit Bond was misplaced in shifting my residence, which is true and correct. 4. That the said original Fixed Deposit Bond was lost by me and I made my best efforts to trace it and it is lost beyond recovery. In case if it is traced in future I shall submit it to the Bank authorities for cancellation, which is true and correct. 5. That now I am in need of Duplicate copy of said Fixed Deposit Bond for collecting my maturity amount, which is true and correct. 6. That the Branch Manager, Sudha Co-op. Urban Bank Ltd. has got ample discretion powers for issuing me duplicate copy of said Fixed Deposit Bond and I request you to kindly issue the same in the interest of justice.
