    SERVICE CONTRACT
    KNOWN TO ALL MEN BY THESE PRESENTS:
    This Service Contract is made and entered into this 3rd day of
    February 2014 at Taguig City, by and between:
    The Technical Education and Skills Development Authority National
    Capital Region-CaMaNaVa District Office (TESDA NCR CAMANAVA), a national government agency with main office at 5 th
    Floor, Victory Central Mall, Monumento, Caloocan City represented
    herein by the Regional Director, MARIFLOR R. LIWANAG and
    herein referred to as the First Party,
    and

    LORNA A. HERRERA, Filipino, 52 years of age, married and


    resident of 15 Knight Street, KC, Mambugan, Antipolo City herein
    referred to as the Second Party.

    WITNESSETH
    That the First Party hires the services of the Second Party as Processing
    Clerk to be assigned at the Regional Office;
    That the Second Party possesses the required education, experience, skills
    and knowledge to perform clerical, administrative and technical duties and
    responsibilities,
    That the First Party urgently needs to hire the Second Party due to exigency
    of the service and there is no existing administrative staff that can perform the work;
    That due to budgetary constraints, the First Party would not be able to hire
    said services under casual or contractual appointment;
    That the Second Party agrees that aside from his salary, no additional
    monetary and non-monetary benefits shall be provided by the First Party and that
    no employee-employer relationship exists between the First and the Second Party;
    That the Second Party hereby attests that she is not related within the third
    civil degree of consanguinity or affinity to the hiring authority, and / or representative
    of the First Party, that she has not been previously dismissed from the government
    service by reason of an administrative offense, and that he / she reached the

    compulsory retirement age of sixty-five (65). Furthermore, the services rendered


    herein are not considered, or will never be credited as government service;
    That the Second Party agrees to render the required services at the TESDANCR, Regional Office for the period February 1 - 28, 2014, unless otherwise
    terminated as specified in number 2.2 of the Department of Budget and
    Management (DBM) Circular Letter No. 2012-20 dated December 18, 2012
    which states that Department Secretaries/Agency Heads shall continually be
    allowed to renew the appointments of personnel on casual status and the
    contracts of personnel under contractual/consultancy/contract of service/job
    order basis for the period that the Department/Agency in undergoing
    rationalization plan activities, but not to go beyond one (1) month after the
    approval of the agencys Rationalization Plan, or 31 December 2013, whichever
    comes earlier, except those personnel whose authorized period of hiring
    should not go beyond 31 December 2012 and those with specific deliverables for
    a set timeframe, subject to pertinent budgetary, civil service, accounting and auditing
    rules and regulations.
    That this contract shall be subject to the issuance of the DBM of the latest
    circular on the renewal of the contract of service of personnel on job order basis;
    That the First Party agrees to pay the Second Party on a daily wage,
    including Saturdays, Sundays and Holidays as may be necessary with approved
    authorization form the First Party at the rate of Six Hundred Eleven Pesos Only
    (Php 611.00) per day;
    That the Second Party is expected to perform the following duties and
    responsibilities;
    -

    Prepares and reconciles NC inventory report


    Assists in the preparation of NC Certificates
    Prepares and updates list of unclaimed NCs
    Assists in the uploading of the RWAC database
    Receives and processes Enrollment and Terminal Reports
    Prepares monthly Enrollment and Terminal Reports
    Perform other functions as may be assigned by the District Director.

    In witness whereof, the parties have hereunto set their hands this 3rd day of
    February 2014 at Caloocan City.
