The document discusses an equipment analysis that identified various risks related to safety, environment, reputation and business impacts. Several recommendations were provided to address issues like inspections, emergency response, fire safety, and corrosion.

Risks identified included potential for single fatality, major/minor injuries, reputational impacts, and financial losses between $100k-1M. Issues like corrosion, erosion, cracking and vibration fatigue were also discussed.

Recommendations included performing inspections, controlling ignition sources, improving signage, ensuring fire equipment availability, practicing emergency procedures, monitoring for gas leaks, and installing automatic suppression systems.

1. Insure that all recommendations mentioned HSE

for Node 7 in the attached part of the HAZOP
study are accomplished for lowering the rank
of risk.2. Control of all ignition sources which
could lead to fire or explosion.3. Safety color
coded signs "Authorized Persons Only" to
reduce the personell exposure factor should
be available.4. All Fire Fighting equipment
availability to be assured for 100%.5.
Availability and calibration of all gas detectors
to be assured frequently, reports of calibration
should be sent to the RBI team for better risk
ranking.6. Emergency response procedures to
be frequently practiced.7. Assure that
evacuation, escape, and rescue plan is
fequently practiced.8. Assure that a
reasonable number of BA set made available
in the affected areas.9. Assign a fire watcher
when hot work/welding is performed, the fire
watcher shall:9.1. Be equipped with a portable
radio for communication with CCR9.2. Have
fire-extinguishing equipment readily available
and be trained in its use.9.3. Be familiar with
facilities for sounding an alarm in the event of
a fire.9.4. Watch for fires in all exposed areas,
try to extinguish them only when they are
obviously within the capacity of the equipment
available, or otherwise sound the
alarm.9.5.Maintain the fire watch at least a
half hour after completion of welding or cutting
operations to detect and extinguish possible
smoldering fires.

for re-design.2. Tie-in F&G detection system Change
with ESD system considering gas-detection
confirmation actions.3. The ESD system, F&G
system, & DCS to be segregated with a
stand-alone configurations.

1. Thorough analysis for the PH of the

water.2.The partial pressure of CO2, pH and
temperature are critical factors, partial
pressures of CO2 should be decreased inorder to avoid low PH condensates which
leads to higher rates of corrosion.

1. Determine the water dew-point temp. of the Operation Guide

feed and outlet gas, and then use the
industrial standard calculations in order to
enhance the internal corrosion rate and due to
CO2 content and water factor.2. Make sure
that all the operating parameters are all within
the design figure with normal operation.3.
Make sure that the ESD and BDV valves
responses are adequate for reducing the
severity of any upcoming accident event.
