1. I, PASUK K-HATHANACHAIKUL BANATAO, of

legal age, Thai Citizen, and with residence at Unit
10AB, Tower 5, Forbeswood Heights Condominium, 26 th
St., Rizal Drive, Crescent Park West, Bonifacio Global
City, Brgy. Fort Bonifacio,Taguig City, Philippines.
A copy of the Birth Certificate is hereto attached

as Annex A, and a copy of the Certificate of
Residency is hereto attached as Annex B.
2. That I am married to VINCENT REX R. BANATAO

likewise of legal age, a Filipino Citizen, and with the last
known address at #321 Phoebe Lane Sunset Estate,
Angeles City, Pampanga.

attached as Annex C and photocopy of Birth
Certificate is hereto attached as Annex D.
3. That I have not been charged or convicted of a

violent or serious felony, whatsoever, to prove thereto,
is the copy of Police Clearance attached as Annex E ,
and a copy of NBI Clearance attached as Annex F.
4. That I have a good moral character and believes in

the principles underlying the Philippine Constitution. I
have conducted myself in a proper and irreproachable
manner during my entire period of residence in the
Philippines, my relation with the Government as well as
with the community in which I am living.
5. That my Filipino Husband, VINCENT REX R.

BANATAO is the owner of a real estate, situated at Unit
10AB, Tower 5, Forbeswood Heights Condominium, 26 th

6. That I am employed as a Procurement Department

Manager by Siam Cement Group since July 16 th 1988
and has expatriated to be working at United Pulp &
Parer Co., Inc. at 9th Floor, Fort Legend Towers, 3 rd Ave.
Cor. 31st St. Bonifacio Global City, Taguig City,
Philippines since January 1, 2003 to the present.
A copy of Certificate of Employment is hereto

attached as Annex H.
7. That I have accomplished and issued yearly

income subjected to final tax declaration for the last
five years.

Payment/Tax Withheld is hereto attached as Annex I.
8. That I have two minor children named, Andrea

Unchalee K-Hathanachaikul Banatao born on March
16, 2002, and Vince Adulramon K-Hathanachaikul
Banatao born on June 27, 2007.
Photocopy of their Birth Certificate is hereto

attached as Annex J and Annex K
9. That I enrolled my minor children of school age in

the following schools recognized by the government.

medical examination, whatsoever.

11. That Ive undergone psychiatric examination
with a certified psychiatrist Dr. Joseph P. Anlacan, and
based on the psychiatric interview and mental status
examination, and aided by the self report questionnaire,
that I am cleared from having any gross psychiatric
condition.

Annex O.
12. That I have been a resident of the Philippines

and I intend to reside herein permanently, to prove as
such, the travel itinerary based on my passport.

Annex P.
13. Attached herewith are the pertinent

documents that would suffice my capability and
capacity for naturalization.
