    PRACTICUM TRAINING AGREEMENT

    I, Kristal Gale C. Camaso, 19 years old, single, with residence and postal
    address at 11-23 Misael St. Diamond Subdivision Balibago Angeles City, Pampanga, a
    bonafide student of Holy Angel University.

    In compliance with the curriculum requirement of my course BS Culinary Arts


    Management, I have to complete 500 hours of on-the-job training / practicum at Hotel
    Euro Asia located at Don Juico Ave., Malabanias, Angeles City, Pampanga, Philippines

    The host establishment has granted me the privilege to undergo practicum and
    agree with the following terms and conditions:
    1. I will be held responsible for my acts during my training.
    2. Holy Angel University and the above mentioned establishment will not be held
    liable for any injury / illness / damages / breakages as a result of my
    negligence that may occur during my practicum-training period.
    3. I will observe the rules of etiquette at all times. I will follow the rules and the
    regulations of the host establishment and all those pertinent to the practicum
    training as discussed by the Practicum Coordinator during the orientation.
    4. I am aware that any violation of the rules and regulations and any form of
    misdemeanor may result to disciplinary action depending upon the gravity of
    the said misdemeanor.
