Introduction:
In 2013, Dr. Alvin Mandapat retired as Head of Education at California USA. He came home here
in the Philippines for good and was burdened to start a Kinder School. He shared his desire to his
relative , Mrs. Ruth Cabal who is an experienced Kinder teacher. After a year Dr. Alvin Mandapat
bought a 2,000 sq. m. lot at Dirita, Iba , Zambales. Together with two other relatives , Mrs. Elizabeth S.
Marave and Miss Vanessa Mae D. Viado Tender Touch Christian School was born. The buildings are now
under construction and we started to compile necessary documents to apply for government permit and
recognition.

1. Board Resolution on the operation of the new course certified by the Corporate Secretary.
For new schools or existing schools that have not yet submitted this document or have change
in the following aspects:
3. Articles of Incorporation and By-laws duly registered with the Securities and Exchange
Commission.
4. Statements on the location of the school in relation to recreation places and other factors
that are not conducive to the learning process.
5. Campus development and landscaping plans.
7. Pictures of the school site buildings, classrooms, laboratories, libraries, medical and dental
health facilities, canteens, sports and recreational areas, etc.
8. Proposed budget/annual expenditures for the succeeding school year approved by the
Board of Trustees.
11. Bond in the amount of one thousand pesos (P1,000.00) per course which should not exceed
P5,000.00 per school irrespective of the number of the courses.
12. Copy of retirement plan of the school registered with the Securities and Exchange
Commission.
13. Copy of the latest financial statement for existing programs certified by an independent
Certified Public Accountant.
16. List of teaching/academic staff of the proposed courses applied for including:
a. Educational qualification, where and when obtained
b. Field of specialization by degree
c. Subject assignments
d. Full-time/part-time
e. Number of teaching/contact per hour
f. Rate of salary per hour
g. Other employee benefits
17. List of laboratory facilities, equipments, supplies and, materials for the courses applied for.
19. Application and inspection fees in the amount of Two Thousand Pesos (P2,000.00)
I, as chairman of the Board of Trustees, promise to maintain the standards required for this
course and to follow faithfully all the laws, rules and regulations, and the requirements of the
Department of Education governing the operation of authorized private school courses and to
inform the DepED of any plan or action regarding closure or phrasing out of the course or any
changes in the prescribed requirements. I acknowledge that any violation of the laws, the rules
and regulations, and the requirements of the Department of Education shall be deemed
sufficient cause for the revocation of the authority granted.
