responsibilities of an On-Call evevt Documenter under the Long Term Agreement (LTA) for on-call
Event Documenters-00039051 - Organizational Effectiveness towards Development Outputs
B) I have also read, understood and hereby accept UNDP’s General Conditions of Contract for the
Services of the Individual Contractors;
C) I hereby propose my services and I confirm my interest in performing the assignment through the
submission of my CV which I have duly signed and attached hereto as Annex 1;
D) In compliance with the requirements of the Terms of Reference, I hereby confirm that I am available for
the entire duration of the assignment, and I shall perform the services in the manner described in my
proposed approach/methodology which I have attached hereto as Annex 3 [delete this item if the TOR
does not require submission of this document];
E) I hereby propose to complete the services based on the following payment rate: [please check the box

led urban poor housing project at Calumpang, Molo, Iloilo City;
Supervised the enumeration of potential 4Ps (Pantawid Pamilyang Pilipino Program) beneficiaries
in Badiangan, Iloilo under the DSWD’s National Housing Targeting System for Poverty Reduction
Project;
Project Facilitator of the community-managed housing project funded by the UK Department for
International Development (DFID) and Swedish International Development Cooperation Agency
(SIDA) which has screened, validated and qualified 21 informal settlers and awarded 21 housing
units;
Facilitated a city-wide community survey of households located in danger zones and disaster
prone and depressed areas in the slums of Iloilo and Bacolod City for the generation of data
utilized as an advocacy and lobbying tool for housing and slum upgrading assistance from the city
government of Iloilo;
Drafted, reviewed and facilitated the signing of the Memorandum of Agreement (MOA) between
the City Government of Iloilo, Hilti Foundation and the Homeless People’s Federation for the
implementation of Balay Kawayan Housing Project which utilized and promoted the use
engineered and treated bamboo for the construction of sustainable and climate resilient houses;
Facilitated the social preparatory activities i.e. community survey, validation and endorsement,
community savings and housing project orientation, free legal consultation for the Usufructuary
arrangement between the ati Tribes Mission and the members of the Ati tribe in Lanit Jaro;
Extended free legal services (including legal consultation, drafting of legal documents, negotiation
and transactions with the land owner) for the prevention of a pending eviction and demolition of
59 informal settler households/families at West Timawa, Molo;
Drafted and submitted funding proposal which successfully accessed a total of ₱4,982,530 bridge
finance for the acquisition of a 4,574 square meter lot in favor of 59 informal settlement families
