    Dear Mr. Rojas,

    The undersigned is seeking for financial and medical assistance to the patient
    MRS., 42 years old and one of the indigent constituents of the City of Passi considering
    that she is the breadwinner for her 2-year old son, her old mother and her husband who is
    unemployed.

    In the year 2010, the patient had undergone chemotherapy as treatment for her
    Breast Cancer, from then on the family earnings are not sufficient for continuous
    medication as part of her treatment. Recently, her doctor, Cherry Pink Andres-Villa, MD,
    FPCP, FPSMO, recommended her to undergo chemotherapy for her spinal column which
    is threatened to be affected by cancer cells. With the required amount of P50,000.00 per
    month for the chemotherapy sessions for six (6) months there is a need for the family to
    come up with a big amount to finish such remedy for her present ailment.

    May this request be given positive and favorable response from your office given
    that any financial and medical assistance that will be granted to Mrs. Castromayor will be
    of great help to complete the medication for her to be back in good health condition.
