    AFFIDAVIT OF DISCREPANCY

    I, APOLONIO S. VILLAS, of legal age, Filipino, married and a resident of


    Barangay Dampalan, Sergio Osmeña, Zamboanga del Norte,, after having duly sworn
    in accordance with law, do hereby depose and state that:

    1. That In the Philippine Statistic Authority (PSA) Records my name was


    incorrectly registered to their system in the name of PRIMO instead of
    APOLONIO;
    2. That APOLONIO S. VILLAS and PRIMO VILLAS is the same person only;

    3. That the above stated discrepancy may be due to clerical error and I learned
    about the PSA records when I requested the records;
    4. As such, I am executing this Affidavit of Discrepancy to attest to the truth of
    the foregoing and to support my daughter’s VISA application.

    AFFIANT FURTHER SAYETH NAUGHT.

    IN WITNESS WHEREOF, I have hereunto affixed my hand this 17h day of


    June 2019 at Dipolog City.
