    AFFIDAVIT OF LOSS

    I, MARJUN DE MANUEL, Filipino citizen, of legal age, Filipino, single, resident


    at Garcia St., Kios Barangay Labangal, General Santos City, after having been
    duly sworn to in accordance with law, depose and state, THAT:

    1. I am a duly-licensed driver in accordance with pertinent Land


    Transportation laws, rules and regulations, and was issued a Student Permit
    License sometime in March 2018;

    2. Last October 2, 2018, upon checking my wallet I noticed that said


    Student Permit License is missing;

    3. Despite diligent efforts, I could no longer locate, find or recover the


    said Student Permit License, and to the best of the my knowledge, the same is
    truly lost;

    4. Said Student Permit License was not confiscated by the Land


    Transportation Office, Police or other Traffic Enforcers for any traffic violation;
    and

    5. As such, I am executing this Affidavit of Loss to attest to the truthfulness


    of the foregoing facts and to support my application for the issuance of a new
    Student Permit License, in lieu of the one that was lost.
