AFFIDAVIT OF EXPLANATION
I, MS. JOSEPHINE B. INES, of legal age, Filipino with office address at Barrio Tambler, Purok Saydala, General Santos City, after sworn in accordance with law, hereby depose and say:
1. That I am the Plant Manager of Alliance Select Foods International Inc., a
duly organized and existing in accordance with law of the Republic of the Philippines;
2. That we have exported our shipment of 1 container containing 3500 cartons of
Canned Yellofin Tuna Chunks in brine “Nautilus” Brand which was exported to Nautilus Food S.A with address at 15 Rue Du Faubourg Montmartre Parts 75009 France under Export Declaration Nos. 48002012 under Bill of Lading No. COSU6106861120 covered by Export Invoice Nos. 1967 dated April 30, 2015;
3. That based on our export bill number COSU6106861120 quantity exported was 3500 cartons and the gross weight is 19393.50kgs;
4. That our buyer, NAUTILUS FOOD S.A rejected subject shipment and returned only 3495 cartons with a gross weight of 19365.30kgs as shown on our import bill of lading number COSU4508481570;
5. That upon filing of the import entry, we are posting a bond equivalent to one and one half (1 ½) times the computed duties and taxes, and obliged to re-export the same within the prescribed period of six months, after it is being liquidated by the Customs Examiner for cancellation of our bond;
6. That the said shipment is not intended for barter, trade and to be processed solely by our company;
7. That we attest that we have not applied for any exemption for this shipment;
8. I am executing this affidavit in compliance with the requirements pursuant to the
provision of Section 105 (d) of the TCCP AS AMENDED. Pertinent to our application for endorsement of duty and tax exemption with the Department Finance.
