    Checklist for Industrial and Sand Gravel Application


    MGB more than5 hectares
    1. Application Form (MGB No. 8-1).
    2. Original Location Map (1:50,000 NAMRIA Topographic Map) showing the
    geographic coordinates (in tabulated form) and boundaries of the proposed
    permit area, duly prepared, signed & sealed by a deputized Geodetic
    Engineer.
    3. Survey Plan or Sketch Plan duly prepared, signed & sealed by a deputized
    Geodetic Engineer.
    4. Five (5) Year Development/Utilization Mining Work Program (MGB Form
    No. 6-2), duly prepared, signed & sealed by a licensed Mining Engineer or
    Geologist. With description of mining method, provide plan/layout and
    pertinent cross section/s showing the details of the development and
    extraction. Design and maps of Screening/crushing facilities signed and
    sealed by Mechanical and Electrical Engineers. And also with Description of
    process plant/mill, provide flow sheet showing the pertinent process and
    equipment, including rated capacity and material balance. Finally,
    Development program, with discussion of activities to be conducted,
    including Gantt Chart of activities showing the budget allocation.
    5. An application (MGB Form No. 16-04) for CEMCRR.
    6. Five (5) Year Environmental Protection and Enhancement Program (EPEP).
    7. Five (5) Year Social Development and Management Program (SDMP).
    8. Safety Health Program.
    9. Proof of technical competence of the preparer (Mining Engineer or Geologist)
    of the Work Program, in the form of:
    -Bio-data and track records in mining operations/environmental
    management or employment contract/proof of availed service(s)
    10. Sworn Affidavit that the Preparer (Mining Engineer or Geologist)of the Work
    Program, that he/she is willing to undertake the project.
    11. Proof of financial capability in the form of:
    - Latest audited Financial Statements (SALN)>10M
    - Income Tax Return (ITR) last three years
    - Credit lines/bank guarantees/deposits>10M deposit
    12. In case of Individual Applicant, Proof of Filipino Citizenship (PSA certified birth
    certificate, Passport, CEDULAand the like).
    13. Endorsements in the form of Resolution from the Local Government Unit/s (LGU),
    Barangay, Municipal or Provincial concerned (2 out 3 endorsements from
    Sanguniang)
    14. Area Clearances from DPWH and NIAthat the project will not adversely affect
    Government structures, infrastructures etc.
    15. National Commission of Indigenous Peoples Certification that the Area applied is
    not within the CADC or CADT of Indigenous Peoples.
    16. Environmental Compliance Certificates (for Extraction and Processing Plant)
    bearing the same Name of Applicant.
    17. Other supporting documents that is/are deemed necessary may be required by the
    Regional Office of MGB.
