    WAIVER OF RIGHTS

    I, JEANETTE O. LOBITAÑA, of legal age, Filipino and with residence address at


    1046-A St. Ignatius Subd., Cabato Road, Brgy. Tetuan, Zamboanga City, hereby
    depose and state that:

    1. I am the compulsory heir of the late ALAIN LU LOBITAÑA (herein referred


    to as the “decedent”) who died intestate on October 13, 2018, a copy of the
    decedent’s Death Certificate is hereto attached and made integral part
    hereof as Annex “A”;

    2. At the time of death, the decedent has death assistance benefit from Value
    Care Health Systems, Inc. registered to the decedent’s name;

    3. I settled the death assistance benefit through the execution of an


    Extrajudicial Settlement of Estate, a copy of which is attached hereto as
    Annex “B”, whereby it was agreed to settle and adjudicate the entire Shares
    in favor of Jeanette O. Lobitaña;

    4. The parties hereto have agreed to indemnify, as they hereby agree to


    indemnify Value Care Health Systems, Inc. as well as its stockholders,
    directors, officers, employees, agents, representatives, assigns and
    successors, free and harmless from any and all liabilities and damages
    arising from or in connection with the execution of this Waiver of Rights, the
    adjudication and the issuance of the said shares in favor of Jeanette O.
    Lobitaña.

    IN WITNESS WHEREOF, I hereby affixed my signature this 7th day of November


    2018 in Zamboanga City, Philippines.
