    AUTHORITY TO DEDUCT

    WHEREAS, my employer, INFORMATION TECHNOLOGY SERVICES MANAGEMENT CORPORATION


    (ITSM), has entered into a Memorandum of Agreement (MOA) with HPP EMPLOYEES MULTIPURPOSE
    COOPERATIVE (HPPEMC), to allow ITSM employees to become Associate Members of HPPEMC and enjoy
    the benefits afforded thereto;

    WHEREAS, I, _________________________________, Filipino, of legal age, with residence at


    ____________________________________________________________, is an employee of ITSM and is
    interested to become an Associate Member of HPPEMC;

    NOW, THEREFORE, upon approval of my membership with HPPEMC, I hereby authorize my


    employer, ITSM, to deduct from my salaries/wages/allowances my HPPEMC deductions including, but not
    limited to, share capital contributions, savings deposits, and loan payments, and to remit the same to
    HPPEMC;

    FURTHER, this is to authorize my employer, ITSM, to deduct from my retirement benefits, last pay,
    and other compensation due me upon my separation/retirement from the Company, any obligations I
    have with HPPEMC, and to remit the same to HPPEMC;

    FINALLY, this Authorization shall continue to be in effect for as long as I am an employee of


    Information Technology Services Management Corporation and a member of the HPP Employees
    Multipurpose Cooperative.
