    INJURY AND LIABILITY WAIVER


    Assumption of Risk and Agreement to Participate

    In consideration of being permitted by the AUSL to participate in the Arellano University School
    of Law Intramurals 2019, I agree to the following waiver and agreement:

    I understand the risk in joining competitive sports in the Arellano University School of
    Law Intramurals 2019.

    I realize that those risks include falls, injuries, any other kinds of similar risks while
    participating or observing the activities during the event.

    I am physically fit and know no medical reason why I should not participate in the
    activities that take place in the Arellano University School of Law Intramurals 2019.

    I hereby voluntarily release the Arellano University School of Law and the institutions
    that will host the event from any and all liability claims, demands, or actions which are related
    to, arise out of, or are in any way connected with my participation in this activity.

    My signature below indicates that I have read this waiver form and that I understand its
    stipulations. I agree to be bound by its terms and conditions.
