    Greetings!

    This is to request for cancellation of the check issued to Philippine Nurses Association,
    Inc. with check number 0000465770 issued on October 2, 2019. The check amounting
    to five thousand seven hundred pesos (₱ 5,700.00) is supposed to be for my
    registration fee on the Philippine Nurses Association, Inc. annual convention which will
    be held in Cebu City. However, the payee name has no Cebu Chapter specified, which
    is indicated on the invitation letter attached to my request. Hence, it will not be credited
    to the host city. Therefore, I decided to pay in cash since the convention date is near to
    avoid problems such as not being able to register successfully. In addition, may I
    respectfully request that the check be under my name instead.
