    JOINT-AFFIDAVIT

    We, GERLYN G. GABELINO and EDGARDO M. FEDELINO, both of legal age,


    Filipinos and residents of Brgy. Granada, Bacolod City, Philippines, after having sworn to
    in accordance with law, hereby depose and state that:

    1. We personally know MARIO DEMONTEVERDE GANTALAO who is a


    resident of Brgy. Granada, Bacolod City and who died on October 20, 2019 at said place.
    We are close acquaintances with said person and are privy to some of his personal
    information and circumstances;

    2. That we know for a fact that Mario D. Gantalao was born on May 2, 1961 based
    on his Certificate of Baptism, attached hereto for reference;

    3. In his Social Security System (SSS), his date of birth was listed as May 2, 1961.
    However, in his Death Certificate, his date of birth was erroneously entered by the
    personnel in charge as May 2, 1962 through inadvertence;

    4. That the true and real date of birth of Mario D. Gantalao is May 2, 1961 as
    reflected in his SSS records and Certificate of Baptism, and not May 2, 1962;

    5. That we are neither related by consanguinity or affinity by said person nor we will
    receive any consideration in executing this affidavit;

    6. Thus, we execute this affidavit to attest to the truth of the foregoing facts for
    whatever legal purposes this affidavit may accordingly serve.

    IN WITNESS WHEREOF, we hereunto set our hand this October 31, 2019 at the
    City of Bacolod, Philippines.
