PETITION
Petitioner, through the undersigned counsel and to this Honorable Court,

most respectfully states that:
1. Petitioner is of legal age, Filipino, single, a resident of 523 Abad

Santos St., Kibawe, Bukidnon where she may be issued Orders and
Processes of the Honorable Court thereat;
2. Respondent Civil Registrar of Municipality of Kibawe is a duly

instituted office responsible for the civil registration program in the
local government unit pursuant to the Civil Registry Law, the Civil
Code, and other pertinent laws, rules and regulations issued to
implement them. It is in-charge of the registration and
cancellation/correction of entries in the civil registrar where the fact
of birth of the said petitioner was registered, its office is located at the
Local Civil Registry Office of the Municipality of Kibawe, Bukidnon
where Orders and other processes of the Honorable Court may be
sent thereat with notice to the Office of the Provincial Prosecutor;

respondents Other Persons are parties in interest who may be
affected by this instant petition and herein impleaded in compliance
to Section 3, Rule 108 of the Rules of Court;
3. Petitioner was born on ”18 September 1993” at Kibawe, Bukidnon

which fact of birth was belatedly registered on May 8, 2003 but
erroneously entered ”18 September 1973” as petitioner’s date of
birth, before the Office of the Civil Registrar of the Municipality of
Kibawe, Bukidnon under Civil Registry No. 2003-386. A duly
authenticated copy of the Certificate of Live Birth of GINALYN
BANGOD BUTLIG is hereto attached as Annex “A” and made
integral part hereof;
4. The age of petitioner’s parents namely Nenita Mijares Bangod and

Felicisimo Capuling Butlig as indicated on the Certificate of Live Birth
were also mistakenly entered as ”21” and ”30” respectively at the
time of petitioner’s birth instead of ”41” and ”50” respectively;
5. Petitioner had been using her correct date of birth ever since in all her
records including petitioner’s school records and Baptismal
Certificate a copy of thereof are hereto attached as Annexes “B”, and
“C” respectively and made integral parts hereof;
6. A Joint Affidavit of Elder Relatives was issued by MERLINDA RULE

BANGOD and FELICISIMO BANGOD BUTLIG, JR. in support to
this petition. A copy of the joint affidavit is hereto attached as Annex
“D” and made integral part hereof;
7. In order to correct the aforementioned misinformation and erroneous

entries in her Certificate of Live Birth, Petitioner files the instant
Petition pursuant to Rule 108 of the Revised Rules of Court;
PRAYER
WHEREFORE, premises considered, it is most respectfully prayed to

this Honorable Court that after due notice and hearing, an Order be issued
directing the herein Respondent Civil Registrar of Kibawe, Bukidnon to

correct the information supplied in item “3. Date of Birth” from ”18
September 1973” to ”18 September 1993”, in item “11. Age at the time of
this birth:” from “21“ to “41“ and in item “17.Age at the time of this birth:”
from “30“ to “50“ in the Certificate of Live Birth under Registry No. 2003-
386 of Petitioner.
Other reliefs consistent in law and equity are also sought.

1. I am the petitioner in the above-entitled case; that I have caused the preparation
of the foregoing petition and have read the allegations contained therein; that the
allegations in the said petition are true and correct of my own knowledge and authentic
records in my possession; I hereby certify that I have not commenced any other action
or proceeding involving the same issues in any court, tribunal or quasi-judicial agency
and, to the best of my knowledge, no such other action or claim is pending therein;
2. If I should learn thereafter that a similar action or proceeding has been filed or is
pending, I hereby undertake to report that fact within five (5) days therefrom to the
court or agency where the original pleading and sworn certification contemplated
herein have been filed; that I executed this verification/certification to attest to the truth
of the foregoing facts and to comply with the provisions of Adm. Circular No. 04-94 of
the Honorable Supreme Court.
