    AFFIDAVIT OF NO DEATH CERTIFICATE

    The undersigned, MARILOU T. MULINGTAPANG, of 53 years


    old, married, Filipino, and presently residing at No. 217 San
    Pascual Street, Barangay Commonwealth, Quezon City, after
    having been sworn to on oath in accordance with the law do hereby
    depose and say:

    1. That herein Affiant is the daughter of the deceased


    ESPERANZA SISON TINDUGAN, of legal age, married to
    Moises Tindugan, Filipino and a former resident of Lingayen,
    Pangasinan;

    2. That both of affiant’s parents already passed away. Their


    death however was not registered at the Local Civil Registrar,
    hence no Certificate of Death can be issued in their favor;

    3. That I execute this affidavit to attest to the truth and veracity


    of the above foregoing and for all other legal purposes it may
    serve.

    AFFIANT FURTHER SAYETH NAUGHT.

    2013, affiant personally appeared before me and exhibited Barangay
