    JOINT AFFIDAVIT OF TWO DISINTERESTED


    PERSONS (re: with Explanantion)

    We, DELIA T. AGUIRRE, and ELIZABETH P. AGUIRRE, both of legal age,


    Filipino, married and residents of Purok 5, Cambanogoy, Asuncion, Davao Del Norte,
    after having been duly sworn to in accordance with law, hereby depose and state:

    That we personally know the person named NESTOR MALAZA AMORIN;

    That we know for a fact that NESTOR MALAZA AMORIN was the father
    of DAVE O. AMORIN;

    That on March 23, 2018, NESTOR MALAZA AMORIN, passed away as


    evidenced by his Death Certificate duly issued by the Office of the City Civil
    Registrar of the City of Maasin;

    That in the Certificate of Death the middlename was erroneously entered


    as “DUMANHUG”;

    That based on the records of Certificate of Live Birth of NESTOR MALAZA


    AMORIN, duly issued by the Office of the Civil Registrar of the Municipality of
    Macrohon, his father’s fullname is recorded as “NESTOR AMORIN” and the
    maiden name of NESTOR MALAZA AMORIN’s mother is “MARGARITA
    MALAZA”;

    That based on the said entry it was clear that the correct middlename of
    NESTOR MALAZA AMORIN is “MALAZA”;

    That the said entry was wrongfully made by a relative in Maasin, Leyte when
    NESTOR MALAZA AMORIN was hospitalized and that the son failed to correct
    the said entry as he habitually resided in Asuncion, Davao del Norte;

    That since time immemorial, WE know the father of DAVE O. AMORIN as


    “NESTOR MALAZA AMORIN”;

    That “NESTOR DUMANHUG AMORIN” as erroneously entered in the his


    Death Certificate and “NESTOR MALAZA AMORIN” in the records of the Social
    Security System (SSS) refers to one and the same person;

    That we are not related to the said NESTOR MALAZA AMORIN by


    consanguinity or by affinity, but we personally know all the aforestated facts
    because we are close friends and neighbors for a long period of time until 2010
    when he decided to reside in Maasin, Leyte with his relatives;

    That we execute this affidavit to attest to the truth of the foregoing facts and
    to inform the proper authorities concerned regarding the matter.
