1.Review the PRHC Falls Prevention patient education pamphlets by November 15th.
2.Review and understand the PRHC Fall Risk Intervention tool by November 15th.
3.Learn the specific needs of each patient I am assigned to by looking at their chart and health history
each shift.
4.Review the facility’s policies and protocols in regards to patients identified as fall risks by November
15th so that I can properly integrate safety measures such as bed alarms, restraints and mobility aids.

I will improve my infection control skills by using the CNO best

How am I going to achieve my goal? Identify a minimum of three (3) specific learning activities with timeframes and timelines.

1.Always being aware of patients on isolation every shift.


2.Review donning and doffing protocols / best practice provided by TFSON by November 15th.
3.Remember to wash hands and use hand sanitizer after exiting a patient’s room who is on isolation by
November 15th.
4.Carrying a few of the PRHC “clean” labels to remind me to wipe down equipment after each use by
next shift.
5.Keeping gloves on me at all times will also help with this goal.
