    The petitioner prays that the following be marked as exhibit “A” the Certificate of Live Birth

    with Registry No. 1234 (I-12) A-N, issued by the Local Civil Registrar of the City of Surigao
    and prepared by Pedro Gonzales, dated April 25, 1994.

    To be marked as Exhibit “B” the Certificate of Live Birth with Registry No. 12-345, issued by
    the Local Civil Registrar of Basilisa, Surigao del Norte (now Dinagat Islands) and prepared by
    Domino Alvarez, dated January 10, 2011.

    To be marked as Exhibit “C” the Certificate of Baptism issued by the Immaculate Conception
    Parish of Nonoc, Surigao del Norte and prepared by Rev. Fr. Mark Anthony Lariosa, DCS, dated
    December 25, 1994.

    To be marked as Exhibit “D” the Official Transcript of Records issued by the Office of the
    Registrar of Northeastern Mindanao Colleges and prepared by Teotima Cruda, dated April 25,
    2015.

    To be marked as Exhibit “E” Social Security System Identification card.

    To be marked as Exhibit “F” the National Bureau of Investigation Clearance issued by NBI

    PROVING OF JURISDICTIONAL FACTS

    May it please the court.

    I’d like to ask the court to mark the following exhibits proving compliance with the
    jurisdictional requirements:

    Exhibit “A” – Petition for Change of Name of Ericson Ocon Cadavis to Earlson Ocon Cadavis
    and Cancellation of Certificate of Birth and Correction of entry in Certificate of Birth, consisting
    of three (3) pages;

    Exhibit “A-1” – Petitioner’s signature in the Verification and Certification of Non-Forum


    Shopping;

    Exhibit “B” – Order of the Court dated January 25, 2020 giving due course and setting the
    petition for hearing;

    Exhibit “B-1” – Notice sent to Office of the Solicitor General;

    Exhibit “B-2” – Notice of Appearance of the Solicitor General with Authority to the Office of
    the City Prosecutor of Cebu City to appear in this case;

    Exhibit “B-3” – Notice sent to Local Civil Registrar of Surigao City;

    Exhibit “B-4” – Notice sent to Local Civil Registrar of Basilisa, Surigao del Norte (now Dinagat
