ORIGINAL CERTIFICATE OF TITLE NO. T-637749
I, ADELINA ABELLO DE GUZMAN, Filipino, of legal age, married to

SAMUEL S. DE GUZMAN, presently residing and with postal address B14 L12
Queensrow Subdivision, Molino, Bacoor, Cavite, after having sworn in
accordance with law, depose and say:
THAT, I am the registered owner of a parcel of land located in Brgy.

Butong, Cabuyao, Laguna under TCT NO. T-637749, containing NINETY EIGHT
(98) SQUARE METERS more or less, issued by the Register of Deeds for
Laguna, Calamba Branch on 26 September 2006.
I always place said title inside a folder which would be usually placed
inside our tokador in our old house in Cabuyao, Laguna. However, in 2015 we
had to move to our new house from Cabuyao to Molino, Bacoor Cavite.
That during this transfer all of our things had to be moved and during
this process we should have included and believed that we had the title with us
when we moved. However, when I was about to pay for its real estate taxes I
wanted to look for the title but I cannot anymore find the title in my effects in
our new house in Bacoor Cavite.
I searched for it and exerted effort and diligence and it donned unto me
that the title might have been lost during the transfer and was disposed off
because of moving and thus my title is considered to be LOST FOREVER.
This affidavit is being executed to attest to the truthfulness of the above

facts and to support the petition before the Regional Trial Court for the
issuance of an Order directing the Register of Deeds of Calamba,
Laguna to issue a REPLACEMENT of the lost ORIGINAL OWNERS
DUPLICATE COPY with the same force and effect as the lost one,
upon payment of the prescribed legal fees;

residing and with postal address at Gateway Ave., B5 L10 PLDT Village,
Brgy. Soro-soro, Binan City, Laguna, after having sworn in accordance with
law, depose and say:
THAT I am the co-owner of a parcel of land technically known as

Lot 3 Blk. 1 of the consol.-subd. plan, Pcs-04-002031, being a portion of
the consol. of Lots 63, 64, 65, 66, 67, 68 & 69, Blk. (LRC) Psd-41919,
LRC Rec. No. ) containing ONE HUNDRED FIFTY (150) SQUARE
METERS, situated in the Bo. of San Antonio, San Pedro City, Laguna,
TCT No. 463713 is attached hereto as Annex “A” made part hereof;
THAT the ORIGINAL OWNERS DUPLICATE COPY of the said Title

was in my possession during the height of Typhoon Typhoon “Ondoy”
which flooded our community and best efforts exerted to locate it
proved futile, hence said Title is considered lost forever;
THAT this affidavit is being executed to attest to the truthfulness of the

above facts and to support the petition before the Regional Trial Court for the
issuance of an Order directing the Register of Deeds for Laguna,

presently residing at 152 P. Arambulo Street, Sta Rosa, Laguna, after being
sworn in accordance with law, depose and say:
THAT sometime on October 17, 2001, RICHEL C. SISON bought a

parcel of land from Philippine National Bank popularly known as Lot 6, Block
2, of the subdivision plan (LRC) Psd-158309, being a portion of Lot 940 Sta
Rosa Estate, L.R.C. Rec. No. 8375) situated in the Bo. of Tagapo, Mun. of Sta
Rosa, Prov. of Laguna, covered by TRANSFER OF CERTIFICATE OF TITLE No.
T-243835 issued by the Register of Deeds of Calamba, Laguna, containing an
area xerox copy of the Title is attached, marked as Annex “A” and made an
integral part of this affidavit;
THAT the original OWNERS copy of said TRANSFER CERTIFICATE OF

TITLE No. T-243835 was misplaced and cannot be located despite diligent
search, hence considered lost;
THAT this affidavit is being executed to attest to the truthfulness of the

above facts and to request the Register of Deeds of Calamba, Laguna to cancel
TCT No. T-243835 and issue a new ORIGINAL COPY OF TRANSFER
CERTIFICATE OF TITLE covering said parcel of land after payment of the
prescribed legal fees and issuance of a judicial order directing the Register of
