1. OBJECTIVES:
The purpose of this document (SAFETY ACTION PLAN) is to ensure that the proactive practices is maintaining within the organization of Nesma
in Marafiq ISS & ACS Project.
2. RESPONSIBILITIES:
Managers (Project & Construction)
Managers shall responsible and committed to the full compliance of the Safety at workplace. Provide safety resources needed in this project.
Supervisors (HSE):
HSE Lead shall take closed monitoring and reporting to management for any safety concerns.
Site Engineers/Site Supervisors:
Engineers, Foreman and Site Supervisors are responsible to take action raised by safety department. Ensure that Safety is the Top Priority in the
workplace.
Safety officers:
Supervise the working area and conduct regular HSE inspection and audits on the site. Keep the records and log all observations and concerns
in the site. Eliminate all imminent danger in the workplace.
APR’s:
Responsible to ensuring that all safety requirements is in place as per PTWC prior to start of work. Implementation of PTW procedure on site.
Employees:
All employees are responsible to their safety and others safety on the site. Follow all safety instructions and report any unsafe act and condition
on the site.
3. TRAININGS
Safety Induction:
All site personnel, before being assigned to its duties shall receive a Marafiq Safety Orientation/Induction as per procedure to ensure his full
awareness of relevant Marafiq safety requirements and rules associated with the job and Plant Prohibition and Regulations.
Internal Trainings:
All field supervisors and skilled crafts personnel shall be developed with programs for Safety Leadership Skill-path training that aim to equip them
with the knowledge and skills to fulfill their duties safely. Safety standards and requirements will be dealt with in detail. Skilled workers, laborers
operators and all Personnel working on site shall receive an internal Training to be conducted by Nesma HSE Department through Nesma HSE
Training Program and Matrix.

4. PRE-TASK BRIEFING
Daily Pre-Start Talks meeting will be used as tools to help modify behavior of the employees to work safely, effectively assign permitted work and
to acknowledge individual workers and crew for working safely. PTB will be carried out by foremen/Supervisors/APR, HSE officer and their work
crew. These daily PTB will have duration of approximately 15 minutes and its contents will be divided into two main areas. The first part of the
PTB will concentrate on general topics, observations carried out during inspections, noticeable incidents and any dangerous occurrences, while
the second part will focus in detail on the work to be executed (description, applicable safety procedures and precautions for the work to be
executed).PTB shall conducted daily.
5. MASS TOOL BOX MEETING
A weekly general safety meeting or more commonly known as ‘Weekly Toolbox Talk’ shall be performed jointly by the Project Operations key
personnel and the Safety Team, among a general assembly of project personnel, usually conducted at the start of a new work-week. This meeting
is aimed to coordinate safety efforts and discuss current safety concerns, action plans, incidents, training needs, security issues and other
relevant safety issues. The discussions cover the following:
Previous weeks safety highlights
Lessons Learned
The safety awareness campaign topic for the current workweek
Announcements, new programs, safety regulations and policies
Discussions may include basic lessons on use of PPE, Material Handling, Housekeeping, Heat stress, Electrical safety, Personnel Hygiene, First
aid, Smoking policy, General safety rules, Emergency evacuation, Use of fire extinguishers, Safe handling of hand tools and power tools etc.
Outstanding safety performances and recognitions may also be included to support any safety incentive program, and serve as an inspiration
and encouragement to project personnel. The Weekly Mas Toolbox Talk shall be documented using a form, which shall include signed
attendances of personnel present. These records shall be filed and made available for Head Office and MARAFIQ review.
6. HSE WEEKLY MANAGEMENT MEETING
A weekly management meeting shall be attended by Nesma HSE Team, Construction or Project Manager with the presence of MARAFIQ/PMC
HSE to discuss weekly safety concerns and the observation result from the weekly walkthrough. All safety deficiencies should be corrected as
soon as possible followed up and promptly closed out within a week. Documentation of corrective actions should be recorded and maintained.
Conditions that present a hazard are to be corrected or controlled immediately. All records of inspections and corrective actions must be
maintained regularly on the project file.
7. HSE WEEKLY MANAGEMENT WALKTHROUGH
Team members from Nesma HSE, Construction/Project Management and from MARAFIQ/PMC HSE shall make a quick round to all sites once in
a week prior to conduct HSE Weekly management meeting to address current safety concerns from personal and visual observation. For this, the
management itself will be aware of the updated situation on the jobsite. Observations identified shall be closed by concerned
personnel’s/Department

8. HSE INSPECTION
8.1 Job Safety Inspection Program
Nesma Safety Team shall monitor all construction activities to ensure that services do not imperil any person or property, and are performed in
conformity with MARAFIQ Loss Prevention policies, principles and practices.
It shall be the duty of the Foreman/Worksite Supervisor to perform daily safety inspection of his assigned worksite. It is expected that the
foreman/supervisor shall perform the weekly safety inspection jointly with the safety officer.
8.2 Inspection of hand tools and power tools
All hand tools & power tools will be inspected by the designated competent person and/or user before and after use and before storage. Any
defective tool will be immediately withdrawn from use, marked "DO NOT USE" and returned to store room for necessary repair or disposal.
Nesma project store keeper will check every returned tool and ensure that defective ones are plainly identified and isolated from others. He will
also ensure that complete records of tools issued, repaired or disposed of is available for inspection.
All hand tools shall be colour coded with the monthly inspection colour coding. Paint, electrical tape or tie-raps could be used on hand tools.
Electrical / power tools shall be colour coded with tie raps or electrical tape at the electrical plug on the cord. Tools that are not inspected and do
bear the valid colour code and tag shall not be used and removed from the work area immediately.
Inspection of tools and equipment shall be carried out by the competent person as per Marafiq check lists and safety manual. Inspection records
shall be maintained for audit
8.3 Inspection of Equipment
All equipments will be inspected by MARAFIQ/MaSa prior to proceed within the Marafiq Premises. Pre-Inspection shall be on daily basis prior to
start the work by operator using the daily checklist form. Any defective equipment will be immediately withdrawn from use, marked "DO NOT USE"
and immediately remove from the site. Nesma shall mark the mobile equipments working within the Marafiq Plant including Equipment ID No.,
Nesma LOGO/Name, and Project Name to recognize by Marafiq and/or MaSa in the area. Nesma HSE ensures that complete records of
equipments including Marafiq Approved Inspection Form and Third Party certificates are maintained and kept.
8.4 Inspection Color Coding
All equipment shall be inspected using the Safety Inspection Table below. The equipment or tool should have a marker to identify that it is part of
the inspection program. A monthly color code system shall be implemented to ensure the equipment or tool has been inspected to be in good
working condition and that it is safe to be used for site operation.

10.2 Responsibility
Project
Shall ensure that all necessary PPE is provided to all Nesma personnel.

Foot Protection: All employees must wear approved work boots or shoes in good condition. Soft shoes, such as tennis shoes and
sandals are not permitted. Boots/shoes should also protect the ankle.
Fall Prevention: as may be applicable, 100% tie- off system of fall arrest devices shall be utilized during ascent, descent and during the
work effort at elevated locations over 6ft or where qualified falling hazards exist.
Ear plug and Ear muff – where applicable, ear plugs must be worn when personnel are involved in or working close to high noise
operations such as air hose cleaning, concrete breaking or cutting or chipping, etc. or when performing activities in areas identified to be
"High Noise Areas" or close to high noise producing equipment such as compressors. It is imperative to select the appropriate device on
the basis of comfort, but more importantly, on the noise attenuation and protection they provide.
Face shields - will be worn when conducting activities such as concrete chipping, grinding or Torch cutting, any activity which requires
face protection, when wearing safety glass
Hand gloves -For all activities requiring hand protection from sharp or rough objects which include: cable pulling, hand digging, rigging,
lifting operations or piping work. Gloves will not be used in situations where the individual is exposed to "pinch points or working close to
moving machinery parts or gears or objects. Welding Leathers (gloves and leggings) will be provided for welders on site. Gloves should
not be worn when working on or with moving machinery.
Disposable dust masks - for all personnel exposed to ordinary respiratory hazards such as; dust and similar air contaminants.
Chemical Cartridge Respirators - where required, during activities such as painting, insulation and similar activities where respiratory
hazards from mist of paints, thinner, organic vapors, minute fiber, etc. exist. For both comfort and safety it is imperative that the
appropriate respirator be fit – tested to the wearer. It should fit snugly and securely.
Chin Straps - for all personnel engaged in elevated work where hard hats may fall from employee’s head and works associated with

If trapped in an office/room:
Place cloth material around/under door to prevent smoke from entering.
Retreat. Close as many doors as possible between you and the fire.

RECORD KEEPING
Nesma will maintain, in format approved by MARAFIQ, an active record showing all:
Work Injuries
Near misses
Incident of property damage as defined by MARAFIQ
Motor vehicle collision
Incident involving damage to MARAFIQ equipment and property
Damage and near misses to heavy equipment and personnel
This record shall be available for inspection at all reasonable times and shall be submitted to MARAFIQ on request.
Incident that results in property damage or serious injuries to personnel and hospitalization of two or more employees must be fully investigated
and reported. Unless the real cause is known, the hazard cannot be controlled in future.
Near miss incident is equally important from the point of view of prevention and should also be thoroughly investigated.
TRAINING FOR IMPLEMENTATION OF INCIDENT REPORTING AND INVESTIGATION
The Safety Officers and Task Supervisors shall have training for both for (1) Implementation of the Incident Reporting & Investigation Program
and (2) Conducting incident reporting & investigation.
Orientations shall also be provided to all concerned employees related to the execution of this program (e.g., project/construction manager,
foreman, project engineer, etc). A qualified reporting and investigation trainer shall be appointed to conduct the training. Training sessions shall
be documented and records of training shall be kept for file reference.
ROOT CAUSE ANALYSIS
A root cause analysis shall be conducted following a major incident, a high potential near-miss or any dangerous occurrence.

3. Fault Tree or Fishbone Analysis to find root cause
4. Reducing risk to ALARP
The above steps will be taken by a team chaired by the HSE Supervisor and project manager and a formal report will be submitted to the Top
Management. Follow up will be done to ensure that the root cause of the incident has been corrected and preventive measures have been
installed or recommended.
INCIDENT REPORTING
All near-miss and injuries (first aid treatment case) must be reported immediately to the first line supervisor/ foreman and NESMA Safety officer.
The MARAFIQ accident/ incident report form shall be completed within 24 hours and must be submitted to MARAFIQ safety representative for any
major cases.
NESMA safety officer shall report all incidents, including near misses to MARAFIQ safety section immediately.

INCIDENTS REPORT
1. First Aid cases will be treated and a record maintained for MARAFIQ review
2. Medical treatment cases will be recorded and submitted to MARAFIQ
Medical cases resulting in RWC/LTI will be reported to MARAFIQ. An immediate oral report will be made by the Safety Supervisor to Nesma
Office and MARAFIQ Representative in the following cases:
All fatal injuries and High Potential Near Misses
All injuries requiring medical attention which results in lost time. (Lost time being defined as a full day’s work lost not counting the day of actual injury)
All damages to contractor’s plant or equipment in excess of SR. 10,000
All damages to MARAFIQ property in any amount and All fires related incident
Damage and near misses to heavy equipment and personnel.
A fully detailed written report of these incidents will be submitted within 24 hours with copy to MARAFIQ Representative. All accidents will be
recorded and monthly injury and damage reports submitted to MARAFIQ. In addition any injury which incapacitates a man for three days, must be
reported in writing & must be submitted to MARAFIQ.
INVESTIGATION PROCEDURE
The scene of an accident must be left undisturbed until Government Affairs, Industrial Security, the Loss Prevention Department and the safety
supervisor have conducted their investigation. In some cases the accident site must be rendered safe so as not to contribute to further accidents.
However precautions shall be taken to keep the accident scene intact as much as is possible, to assist local police in their investigation. The
