I, NESTOR GIZREL CARDENAS ONG, of legal age, Filipino and a resident
of Fortich St., Barangay 2, Malaybalay City, Bukidnon, after having been duly sworn
to according to law hereby depose and state:
That I am the registered owner of a certain business establishment duly

registered with the Department of Trade and Industry per Original copy of
Certificate of Business Name Registration attached hereto and described as
follows:

That I want to CANCEL my business name registration;
That the cancellation is not intended to defraud creditors and that registered
BN owner has no outstanding and pending financial obligation in connection with his
business or that he has notified all the creditors of such cancellation, as the case
may be.
That this Affidavit is executed by me for the purpose of attesting the true facts
above stated and to request the Department of Trade & Industry to issue a
Certificate of Business Name Cancellation and for whatever legal purpose it may
serve.

I, NESTOR GIZREL CARDENAS ONG, of legal age, Filipino and a resident
of Fortich St., Barangay 2, Malaybalay City, Bukidnon, after having been duly sworn
to an oath in accordance with law depose and say:
1. That I am a registered owner of BUSY BEAN CAFÉ, located at Barangay

2, Fortich St., Malaybalay City, Bukidnon;
2. That I had ceased the operation of the said business since January 31,
2020 due to change/transfer of residence address;
3. That I am executing this affidavit in order to attest to the truth of the

foregoing circumstances and for whatever legal purpose it may serve.

Purok 4, Barangay 1, Malaybalay City, Bukidnon, to be my true and lawful attorney-in-
fact, to manage and conduct all my affairs, and for that purpose in my name and on my
behalf to do and execute all or any of the following acts, deeds and things, to wit:
1. To transact and facilitate any transaction relative to the termination/closure of

my business, BUSY BEAN CAFÉ, located at Barangay 2, Fortich St.,
Malaybalay City, Bukidnon with the office of Bureau of Internal Revenue
(BIR), Department of Trade and Industry (DTI) and any other concern
offices/agencies;
2. To make, sign, execute, process, submit, follow-up, deliver and receive any
document related thereof under such terms and conditions as my attorney-in-
fact may consider or deem proper.

authority whatsoever requisite or necessary or proper to be done in or about the
premises, as fully to all intents and purposes as I might or could lawfully do if personally
