    AFFIDAVIT OF REQUEST

    I, ____________________________, after having been sworn to in accordance


    with law do hereby depose and state that:

    1. That I am a registered co-owner of a property covered by Tax


    Declaration No. ____________________located at Bari, Mangaldan, Pangasinan;

    2. That the said property is being used as a private educational


    institution. Attached as “Annex A” is the picture where the school gymnasium
    is located/built;

    3. That I am executing this affidavit to request that the above


    described property be exempted from realty tax pursuant to Article 325 (B) of
    the implementing rules and regulations of the Local Government Code of 1991;

    4. That I attest to the truthfulness of the above facts and I am executing


    this affidavit for whatever legal purpose it may serve.

    IN WITNESS WHEREOF, I have hereunto affixed my signature this
