Separation of Adjacent

Fields
Field separation - GAPS
 Goal is to achieve dose uniformity when two
beams come together either in the same plane
such as Mantle and Para-Aortic, or in
orthogonals or same plane with Craniospianl
 Gaps are not applied when using the central axis
to match field such as in the breast and
supraclav.
 If non uniform – creates a hot spot or cold spot
causing tumor recurrence

 Calculated with the geometric divergence of
the beam

 A calculated measurement between two


fields, so the geometric edges of the beam
come together at midplane of the patient or at
a specified depth

1. Figure out the depth at which you would like
the gap to be located (where do we want the
two beams to meet)

2. Determine the field size for both fields. Will


most likely use the length of each field (often
length size, not width)

3. Must know the SSD each field will be set up


to.

Gaps
 The longer the field length, the larger the gap
(due to divergence)

 The deeper the depth you are calculating to,


the larger the gap
Gap Calculation
 A patient is being treated to 2 adjacent spine
fields. The field size on the superior field is 7
cm wide x 12 cm long. The collimator setting
for the inferior field is 9 cm wide x 20 cm
long. Both fields are set up at 100 cm SSD.
Calculate the gap on the skin surface if the
fields should abut at 5 cm depth.
Orthogonal junction
 Matching a lateral brain with a upper spine
field.

 Collimator will need to turn to match the


divergence of the upper spine field or
matching the upper border of spine with the
divergence of the lateral field
