Analysis: The table shows the presence of breeding or resting sites of vector disease. The breeding site presents are containers, plastic drums, waste disposal and open reservoir. Flies and
mosquitos and coackroaches were observed. The plastic drums and containers have mosquito eggs.

Falls(staircase, ladder, bathrooms, wet floors) Upon entering their house, there is a stair that slanted. If you go to the bathroom (2 bathrooms) there
is a hole in between. Since the location of the bathroom is too dark, and you are not aware of it, you
might fall.

Poison(cleaning products, pesticides) There are no cleaning products scattered.


Fire hazards(cigarette butt, candles, cooking, electric wirings) The electrical wirings are reachable by children. Also its wirings are knotted.

Sharp objects(presence of knives, scissors, rake, saws) Knives and scissor are not kept properly. You can see it in on their table and it can be reach by children.

Suffocation (presence of cords, strings, ropes, trash bags, electric outlet unfix The rope is hanging on the wood and it can be reach by children. Presence of cords are not properly fix
and it is too low that can be reach by children.
Analysis: The table shows the presence of accident hazard in their house which includes their staircase that slanted, electring wirings that were not properly fix, knives and scissors that were
not put on its proper places, rope that are hanged that can be reach by children, and electric outlet unfixed.

ANALYSIS: Mr. Leonardo Ratuita delivered via normal vaginal spontaneous delivery on March 03, 1950. Mrs. Evelyn Ratuita was also delivered via normal vaginal spontaneous delivery and born
on September 21, 1957. Their daughter, Ms. Loida Ratuita was delivered via Cesarean section on November 16, 1995 at the MMMH-MC. Mr. Joey Tagatac was delivered via normal vaginal
spontaneous delivery on December 09, 1989. LJ was delivered via Cesarean section was born on June 19, 2018at the MMMH-MC.
