    AFFIDAVIT OF GUARDIANSHIP

    I, FELIXBERTO M. JARO of legal age, widower, Filipino, and resident of


    Purok 7, Canaway, Iligan City, after having been duly sworn to in accordance
    with law, depose and state:

    1. That I am the father of Jeralene Tabaranza Jaro, born on May 29, 1981, who
    has the interest in the proceeds of insurance on the life of the late Leah R.
    Tabanranza-Jaro, my deceased wife;

    2. That I am the guardian of the above-mentioned person;

    3. That the said person is under my care and custody since she is a disabled
    person;

    4. That I am competent to receive on behalf of my disabled child the


    proceeds of insurance of the deceased, as well as other benefits and
    amount due her;

    5. That I am not imbecile, insane, a vagrant or a vicious person or a habitual


    drunkard or a habitual criminal, and have not abandoned, neglected or
    refused to support said minor or caused her to commit offenses against
    the law;

    6. That I am executing this affidavit to attest and confirm the foregoing


    facts and to prove that I am the legal guardian of the above-named
    person and for whatever purpose it may serve.

    IN WITNESS WHEREOF, we have hereunto set our hands this 27 th day


    of February, 2020 in Iligan City, Philippines.
