    In accordance with Revenue Regulation No. 3-2005, EO#398 and Revenue


    Memorandum Order No. 10-2006, RR#10-2012, RR#10-2014, RR#1-2012 and/or RR10-2007
    prescribing the implementation of the Electronics Filing and Payment System (EFPS), I would
    like to inform your good Office of my company’s intention to enroll in the Electronics
    Filing and Payment System (EFPS).
    The purposes of the enrolment are:
    -Our organization is a National Government Agency/Corporation
    -Our company would like to volunteer to utilize the EFPS

    I also intend to enroll in the electronic payment system of EFPS-Accredited Agent Bank
    (LandBank of the Philippines) immediately or within a week after ‘On-line enrolment’, and
    Activation by the BIR-TSS Chief. I am submitting herewith copies of DTI Certificate;
    Updated/Latest Certificate of Registration (COR-BIR Form 2303); and/or BIR Form 1905
    (Taxpayer Registration Update form) to include Tax Form/s in my tax return compliance. I am
    also authorizing Miss Ellyza M. Cunanan to undertake the EFPS Transactions.
