    AFFIDAVIT OF DAMAGED PASSPORT

    I, CRISTINA GONZALES KUMANO, of legal age, Filipino, married and with


    address at 3609 Brgy. San Juan I, City of General Trias, Cavite, after having been duly
    sworn to in accordance with law, depose and state:

    1. That I am a holder of a Philippine Passport with number ____________ which was


    issued on ____________________ valid until ______________ and issued at DFA
    ______;

    2. That the said passport was damaged on or about _________________,


    ________________(reason)____________________________;

    3. The damage caused to my passport was inadvertent and unintentional;

    4. I am executing this affidavit to declare the factual circumstances as to how how the
    said passport was damaged beyond further use and to support my present application
    for a new passport to replace the one that is damaged; and

    5. Each and all of the facts stated here are true and correct, and made with full
    awareness that I may be held liable for perjury and have the passport issued in my
    name cancelled if any misrepresentation and/or falsehood are found to have been
    made in this affidavit.
