    RE: CERTIFICATION OF RICHIE C. FERNANDEZ

    I, RUCELLE FLORES VALENCIA, of legal age, single, Filipino citizen, with


    residence and postal address at No. 13 Apollo St., Tivoli Greens Subd. Diliman Quezon City,
    after having been duly sworn to in accordance with law do hereby depose and say:

    1. That I am the authorized representative of RICHIE C. FERNANDEZ and can


    attest that the following information below are true and correct as per attached genuine
    copy of her record;

    2. That my sister-in-law RICHIE C. FERNANDEZ was a bonafide student of


    Chemical Engineering in the school of Chemical Engineering and Chemistry at the
    Mapua Institute of Technology ;

    3. That likewise she was admitted as a regular student to the Chemical Engineering
    program in the year 1997 with student No. 1997127911 and was conferred the degree of
    Bachelor of Science in Chemical Engineering on April 29, 2002

    4. That attached herewith copy of her Certification;

    5. That I hereby attest to the authenticity and genuineness of the document attached
    and I hereby agree to be liable and answerable to perjury and /or falsification of public
    document in the event that the document is found not in order; and

    5. That I am executing this affidavit to attest to the truth and veracity of the above
    foregoing facts and for all other legal and purposes it may serve.

    IN WITNESS WHEREOF, I hereunto set my hand and sign on this ____ day of June
