    COMMISSION BASIS

    SERVICE AGREEMENT

    KNOW ALL MEN BY THESE PRESENTS:

    This CONTRACT made and executed by and between:

    ____________________, owner/proprietor of ___________________________


    FUNERAL with principal office at _________________________¸ hereinafter referred
    to as the "THE ESTABLISHMENT”;

    - and -

    ____________________, of legal age, married and with residence/postal address


    at _________________________, hereinafter referred to as the "THE SERVICE
    PROVIDER;

    Parties agree on the following;

    TERMS and CONDITIONS:

    1. The service provider admits that he started working with THE


    ESTABLISHMENT sometime in mid-2019 when it started operating its
    branch in _____________________;

    2. The SERVICE PROVIDER shall be paid an allowance of Php 1,000 per month
    and TEN PER CENT (10% ) COMMISSION for every client or Service
    Contract referred to THE ESTABLISHMENT;

    3. The SERVICE PROVIDER is not allowed to make the funeral parlor as his or
    his family’s living quarter;
    4. The SERVICE PROVIDER shall be paid additional fee per service if he serves
    as helper during the burial services;
    5. There is no employer-employee relationship between the ESTABLISHMENT
    and the SERVICE PROVIDER and the relationship is more of a business
    relationship with sales commission per funeral service contract.

    For and in consideration of the mutual covenants and agreements herein agreed
    upon, the parties by these presents, have entered, as they hereby enter, into a service
    agreement
