A Clinical Case Scenario

Mrs. D.S., a 20 year-old G1 P0 at 20 weeks AOG came to the health center


for the first time for her prenatal check-up. Her last menstrual period was May 26,
2020. Her husband commented that she was hesitant to visit the health center
since she does not feel anything wrong with her pregnancy. Furthermore, she
claimed that she has not taken vitamins nor any other supplements yet from the
time she learned that she is pregnant. Upon initial assessment, vital signs readings
and other findings were as follows;

BP – 100/60 mmHg

T – 36.8 C

P – 90 beats/min.

R – 20 breaths/min.

Weight : 66 kg Height : 5’6”

FHT – 130 beats/min at the RLQ

Fundic Height – at the level of umbilicus

Based on the case scenario ;

1. What are the significant assessment findings that you have noted?
2. Identify at least 2 priority nursing problems and formulate appropriate
nursing diagnoses for each.
3. Formulate a Nursing Care Plan and a Health Teaching Plan based on the
priority health problem identified.
