    We, JULIET MASALTA GIDA, CIRILA LOPEZ MASALTA, DELIA


    MASALTA NAJARO, all Filipinos, of legal ages, and residents of Matungao,
    Socorro, Oriental Mindoro; Talisay, Cabilao, Loon, Bohol; and Don Cesar
    Cacho Street, Germinal Pob., Bolinao, Pangasinan, respectively, after having
    been duly sworn to in accordance to law, hereby depose and say:

    1.)That we are the legal heirs of the late ROBERTA L. MASALTA, who
    owns a savings account at FCB Loon;

    2.)That said ROBERTA L. MASALTA died October 24, 2019 at Talisay,


    Cabilao, Loon, Bohol;

    3.)That we, the co-heirs hereby name, constitute, and appoint CIRILA
    LOPEZ MASALTA to be our true and lawful Attorney-in-Fact, for us and
    in our name, place, and stead, to do and perform the processing for
    the closure of the passbook of the above-mentioned deceased;

    4.)That we are executing this affidavit freely and voluntarily to the best of
    our knowledge and belief attesting to the truth of the foregoing
    statements and for whatever legal purposes this may serve.
